Late MitraClip procedure after left atrial appendage occlusion
We report the first human case of percutaneous edge‐to‐edge mitral valve repair using MitraClip System (Abbott Vascular, Abbott Park, IL) in a patient, who previously underwent left atrial appendage closure with PLAATO™ System (ev3. Inc., Plymouth, MN). The procedure was successfully performed using the standard MitraClip technique, in the catheterization laboratory, with transesophageal echocardiographic and fluoroscopic guidance. We showed that a double percutaneous procedure for stroke prevention and mitral regurgitation treatment might be a valid approach in selected patients ineligible for chronic anticoagulatio...

Invasive Hemodynamic Characteristics of Low Gradient Severe Aortic Stenosis Despite Preserved Ejection Fraction
ConclusionsOccurrence of low gradient severe aortic stenosis despite preserved ejection fraction was confirmed by invasive hemodynamics and was not the result of a systematic bias in the echo calculation of aortic orifice area.

Volume‐Outcomes relationship in the era of modern coronary intervention ‐ Results from the prospective multicenter German DES.DE registry ‐
Conclusions: In the era of modern coronary intervention technological advances such as DES have not offset the inverse relation between procedural volume and both in‐hospital and 1‐year outcomes after PCI. © 2013 Wiley Periodicals, Inc.

Abstract 340: Improved Patterns for Advanced Non-Invasive Diagnostic Testing Using a Personalized Gene Expression Score among Patients Presenting to Primary Care Clinicians with Symptoms of Suspected Obstructive Coronary Artery Disease: Results from the IMPACT-PCP (Investigation of a Molecular Personalized Coronary Gene Expression Test on Primary Care Practice Pattern) Trial Poster Session III
Conclusion: The GES was associated with a statistically significant and clinically relevant change in clinical decision making among pts evaluated for suspected symptomatic CAD. In conclusion, the addition of the GES showed clinical utility above and beyond conventional decision-making by optimizing the pt’s diagnostic evaluation, particularly around the reduction in the intensity of diagnostic testing among low GES patients.

Atrial fibrillation among Medicare beneficiaries hospitalized with sepsis: Incidence and risk factors
Background: Newly diagnosed atrial fibrillation (AF) during severe sepsis is associated with increased risks of in-hospital stroke and mortality. However, the prevalence, incidence, and risk factors associated with AF during the sepsis syndromes are unclear. Methods: We identified patients with preexisting, newly diagnosed, or no AF in a nationally representative 5% sample of Medicare beneficiaries hospitalized with sepsis between 2004 and 2007. We identified multivariable-adjusted demographic and clinical characteristics associated with development of newly diagnosed AF during a sepsis hospitalization. Results: A total of 6...

Sex differences in clinical outcomes in patients with stable angina and no obstructive coronary artery disease
Conclusions: Women with stable angina and nonobstructive CAD are 3 times more likely to experience a cardiac event within the first year of cardiac catheterization than men. A prospective trial to examine the impact of medical therapy on MACE in patients with nonobstructive CAD is warranted.

Immediate rescue operations after failed diagnostic or therapeutic cardiac catheterization procedures
CONCLUSIONS With rapid transfer to an operation room, minimizing the time of warm myocardial ischaemia, and by performing complete coronary revascularization, it is possible to obtain equally low operative mortality in patients with life-threatening cardiac catheterization-associated complications, as is the case with open cardiac operations in general.

Impact of advanced age on the safety and effectiveness of paclitaxel‐eluting stent implantation in patients with ST‐segment elevation myocardial infarction undergoing primary angioplasty
ConclusionsFor STEMI patients undergoing primary PCI, the implantation of PES as compared with BMS reduced ischemic TVR, and this effect was independent of age. [NCT00433966]. © 2013 Wiley Periodicals, Inc.

Angina Pectoris or Myocardial Infarctions, Pulmonary Arteriovenous Malformations, Hereditary Hemorrhagic Telangiectasia, and Paradoxical Emboli
In conclusion, we suggest that in patients with untreated PAVMs, cardiac ischemia can occur because of a paradoxical embolus through PAVMs to a coronary artery.

Systemic Hypertension in Low Gradient Severe Aortic Stenosis with Preserved Ejection Fraction.
CONCLUSIONS: Systemic hypertension in LG severe AS with preserved EF is associated with elevated LV filling pressures and pulmonary hypertension. Treatment of hypertension with vasodilator therapy results in a lowering of the total LV afterload, with a decrease in LV filling pressures and pulmonary artery pressures. These findings have important implications for the management of patients with LG severe AS with preserved EF and hypertension. PMID: 23956211 [PubMed - as supplied by publisher]

Successful closure of residual leak following LARIAT procedure in a patient with high risk of stroke and hemorrhage.
Abstract Percutaneous left atrial appendage suture ligation with the LARIAT® device (Sentre HEART, Redwood City, California) was successfully performed on an 84‐year‐old woman with non‐valvular atrial fibrillation who developed intracranial hemorrhage on warfarin. However, a large gap developed at follow‐up precluding warfarin cessation. To the best of our knowledge, this case report represents the first description of the use of a Gore® Helex® Septal Occluder (W. L. Gore and Associates, Newark, Delaware) as a novel Removed: and effectiveapproach to close a gap following percutaneous LAA suture ligation. © 2013...
