Clinical Features and Risk Factors of Mortality in Patients with Posterior Reversible Encephalopathy Syndrome
CONCLUSIONS: Our results indicate that PRES could be considered as a sign of a very high-risk patient. We also demonstrated that heart failure and stroke were independent risk factors for death in patients with PRES; moreover, pregnancy was a protective factor.PMID:36419591 | PMC:PMC9678458 | DOI:10.1155/2022/9401661

Frequency of and risk factors for poor cognitive performance in hemodialysis patients
Conclusions: There is a high frequency of poor cognitive performance in hemodialysis patients, primarily affecting executive function. Risk factors for worse executive function include vascular risk factors as well as vascular disease. Normal performance on the MMSE does not preclude impaired cognitive function, because individuals with MMSE score ≥24 also have a high frequency of poor cognitive performance.

Astrocytic Ca(2+) waves mediate activation of extrasynaptic NMDA receptors in hippocampal neurons to aggravate brain damage during ischemia.
Abstract Excitotoxicity plays a central role in the neuronal damage during ischemic stroke. Although growing evidence suggests that activation of extrasynaptic NMDA receptors initiates the neuronal death, no direct evidence demonstrated their activation during ischemia. Using rat hippocampal slices, we detected oxygen-glucose deprivation (OGD) induced slow inward currents (SICs) mediated by extrasynaptic NMDA receptors in CA1 pyramidal neurons. Moreover, Ca(2+) chelator BAPTA dialysis into astrocytic network decreased the frequency of OGD induced SICs, indicating the activation of extrasynaptic NMDA receptors depe...

Stroke Rounds: Warfarin No Help for Dialysis Patients
(MedPage Today) -- The risk-benefit balance did not appear to be favorable for warfarin in patients with atrial fibrillation who were undergoing dialysis, a retrospective study showed.

Lacunar Infarct Presenting Like Axillary Nerve Lesion: A Case Report (P6.250)
Conclusion: Our case was thought like an axillary nerve lession primarily as a result of history and physical examination. But after the tests our diagnosis was an infarct. We can’t find affected only proximal upper extremities like axillary nerve lesion in the literature. We want to point to lacuner infarct that can be presented like peripheral nerve lesion.Disclosure: Dr. Oz has nothing to disclose. Dr. Tasdemir has nothing to disclose. Subarachnoid Hemorrhage as the Initial Neurologic Manifestation in Thrombotic Thrombocytopenic Purpura (P6.014)
Conclusion: Upon review of the literature, ischemic infarction and intracerebral hemorrhage are uncommon complications of TTP. Our case demonstrates a unique case of subarachnoid hemorrhage as the initial neurological manifestation of TTP.Disclosure: Dr. Wei has nothing to disclose. Dr. Ramesh has nothing to disclose. Patients' Characteristics Affect the Survival Benefit of Warfarin Treatment for Hemodialysis Patients with Atrial Fibrillation. A Historical Cohort Study
Background: Stroke prevention in dialysis-dependent patients with atrial fibrillation (AF) is an unresolved clinical dilemma. Indeed, no randomized controlled trial evaluating the efficacy and safety of oral anticoagulants in this population, has been conducted so far. Observational research on the use of warfarin in patients on dialysis has shown conflicting results. This uncertainty is mirrored by the wide variations in warfarin prescription patterns across centers. We sought to evaluate the association between the use of vitamin K antagonists (VKAs) and mortality among hemodialysis patient with AF and to assess potentia...

Elevated ventricular CSF lactate within the first week after aneurysmal subarachnoid hemorrhage is associated with poor outcome (P5.068)
Conclusions:Elevated CSF lactate drawn within the first week of hospitalization following aSAH correlates with outcome. Larger prospective studies are needed to test the validity of this data for predicting outcome and for understanding the pathophysiologic mechanisms.Study Supported by: No funding was received for this study.Disclosure: Dr. Renfrow has nothing to disclose. Cause-Specific Mortality in Patients with Chronic Kidney Disease and Atrial Fibrillation
Conclusions: In a non-dialysis-dependent CKD population, the presence of AF was associated with higher all-cause and cardiovascular mortality. These data suggest that patients with both CKD and AF are at high cardiovascular risk, and thus clinical practice (or trials) should aim at reducing the overall excess cardiovascular mortality (not stroke alone) in patients with AF and CKD.Am J Nephrol 2018;48:36 –45

An unusual combination of neurological manifestations and sudden vision loss in a child with familial hyperphosphatemic tumoral calcinosis
We report a child with hyperphosphatemia who presented with multiple episodes of neurologic dysfunction involving lower motor neuron facial nerve palsy along with sequential visual loss. He also had an episode of stroke. There was an extensive metastatic calcification of soft tissue and vasculature. Hyperphosphatemia with normal serum alkaline phosphatase, calcium, parathyroid hormone, and renal function was noted. He was managed with hemodialysis and sevelamer (3 months) without much success in reducing serum phosphate level, requiring continuous ambulatory peritoneal dialysis (3 years). Intact fibroblast growth factor 23...

Sustained Low-Efficiency Dialysis is Associated with Worsening Cerebral Edema and Outcomes in Intracerebral Hemorrhage
ConclusionsSLED treatment in ICH patients with CKD was associated with significant increase in rate and frequency of PHE expansion. Absolute increase in PHE during 48-h post-ICH was associated with increased mortality and worse functional outcomes. Further prospective and multicenter evaluation is needed to differentiate the effects of RRT on hematoma dynamics and patient outcomes from those attributed to CKD.
