Care of the TIA patient
I read with interest Al-Khaled et al. study on the prognostic impact of the stroke unit (SU) concept in patients presenting after a transient ischemic attack (TIA). They found no significant impact of SU care as compared to conventional care (CU) in TIA patients. Their study raises some interesting observations. The conventional teaching is that patients with TIA warrant emergent evaluation to identify and modify their stroke risk factors. Discussion These results indicate that variation exists in the type of CVD events that occur in breast cancer patients receiving AIs in comparison to tamoxifen users. For example, the risk of ischemic disease or stroke was not elevated in those who used AIs only versus TAM users. However, overall CVD events were greater in women who used AIs only (or sequentially after TAM), especially if they received left-sided breast irradiation. By reducing out-of-pocket costs for preventive treatment, the Affordable Care Act appears to have encouraged more people to have health screenings related to their cardiovascular health,a UCLA study found. Costs associated with renal and cardiovascular events among patients with type 2 diabetes mellitus and nephropathy: a cost model based on the CREDENCE clinical trial.

Process of care compliance is associated with fewer diabetes complications.

Potential of risk-based population guidelines to reduce cardiovascular risk in a large integrated health system.
CONCLUSIONS: The risk calculator provided risk estimates in most patients and demonstrated fairly good discrimination and predictiveness. The risk-based decision tool identified high-risk patients for treatment not identified by traditional guidelines, as well as low-risk patients for whom treatment may be unnecessary. Managed Care after Acute Myocardial Infarction (MC-AMI) improves prognosis in AMI survivors with pre-existing heart failure. A propensity score matching analysis of Polish nationwide program of comprehensive post-MI care
