ASL and susceptibility-weighted imaging contribution to the management of acute ischaemic stroke
AbstractMagnetic resonance imaging (MRI) plays a central role in the early diagnosis of cerebral vascular events. Today, MRI is used not only for the detection of acute ischaemic lesions, but also to fine tune the diagnosis and improve patient selection for early therapeutic decision-making. In this perspective, new tools such as arterial spin labelling (ASL) and susceptibility-weighted imaging (SWI) sequences have been developed. E-176 Endovascular stent retriever thrombectomy of medium and small vessel occlusion of the middle cerebral artery with acute ischemic stroke: a case series
ConclusionsOur case series demonstrated that SRMT in SVO with disabling NIHSS is feasible and safe if performed carefully. All patient achieved TICI 3 perfusion and first pass effect was better observed in trap technique and use of long solitaire STD. Good function outcomes were achieved in all cases irrespective of the types of SRD used and passes required to achieve TICI 3 perfusion. Further studies are required.Disclosures Y. Lodi: None.

Blood pressure and cerebral ischemia: A continuing dilemma
Classical neurologic teaching suggests that a higher cerebral systemic blood pressure (BP) be targeted for individuals with occlusive carotid disease, in order to improve cerebral perfusion through collaterals. This conflicts, however, with observational, population-based studies and clinical trials supporting better outcomes associated with lower BPs. In this issue of Neurology®, Powers et al.1 evaluate nonsurgical controls from the Carotid Occlusion Surgery Study (COSS), and report lower rates of stroke recurrence in individuals maintained at a follow-up BP of 130/85 mm Hg or lower.

Comparison of Levosimendan versus Dobutamine in Patients with Moderate to Severe Left Ventricular Dysfunction Undergoing Off-pump Coronary Artery Bypass Grafting: A Randomized Prospective Study.
CONCLUSIONS: Levosimendan was associated with statistically significant increase in indices of contractility (CI, LVSWI, and RVSWI) and decrease in PCWP during and after OPCAB. Levosimendan group had lower incidence of atrial fibrillation, shorter length of ICU, and hospital stay. PMID: 28393781 [PubMed - in process]

A 69-year-old man developed acute-onset confusion and hypertension with systolic pressures in the 160s 1 day after carotid endarterectomy for right facial droop from left hemispheric lacunar infarcts. CT perfusion (figure, A–D) demonstrated findings consistent with cerebral hyperperfusion syndrome (CHS) following revascularization. Proprietary Software Enhancement of Digital Subtraction Angiography for Perfusion Mapping of Posterior Circulation (P6.022)
Conclusion: AngioViz DSA enhancement technique may reliably identify patients with intra-procedural neurovascular perfusion deficits for posterior circulation when none exists. Effects of Normothermic Machine Perfusion Conditions on Mesenchymal Stromal Cells
In this study the effect of NMP perfusion fluid on survival, metabolism and function of thawed cryopreserved human (h)MSC and porcine (p)MSC in suspension conditions was studied. Suspension conditions reduced the viability of pMSC by 40% in both perfusion fluid and culture medium. Viability of hMSC was reduced by suspension conditions by 15% in perfusion fluid, whilst no differences were found in survival in culture medium.
