The American Heart Association - American Stroke Association and Medtronic Collaborate to Reduce Recurrent Strokes
The American Heart Association/American Stroke Association (AHA/ASA) and Medtronic plc (NYSE: MDT), today announced a collaboration to reduce the rate of recurrent strokes in the U.S. The two organizations will work together over several years to educate, raise awareness and support effective management of patients who have strokes, one of the most devastating diseases affecting Americans today. The initiative, announced at the American Stroke Association's annual International Stroke Conference in Nashville, Tennessee, will focus on reducing strokes of unknown cause, called "cryptogenic stroke."

Thrombectomy for acute ischemic stroke: an evidence-based treatment
Just 2 years ago, at the International Stroke Conference in Honolulu, Hawaii, USA, the Interventional Management of Stroke (IMS III),1 Mechanical Retrieval and Recanalization of Stroke Clots Using Embolectomy (MR RESCUE),2 and SYNTHESIS-Expansion3 trials were presented and concurrently published in The New England Journal of Medicine. The overarching message to the medical community and lay press was that these studies provided unambiguous evidence that mechanical thrombectomy was ineffective in treating acute stroke secondary to emergent large vessel occlusion (ELVO). The investigators were careful to emphasize the a...

Segmentation of cerebrovascular pathologies in stroke patients with spatial and shape priors.
Authors: Dalca AV, Sridharan R, Cloonan L, Fitzpatrick KM, Kanakis A, Furie KL, Rosand J, Wu O, Sabuncu M, Rost NS, Golland P Abstract We propose and demonstrate an inference algorithm for the automatic segmentation of cerebrovascular pathologies in clinical MR images of the brain. Identifying and differentiating pathologies is important for understanding the underlying mechanisms and clinical outcomes of cerebral ischemia. Manual delineation of separate pathologies is infeasible in large studies of stroke that include thousands of patients. Unlike normal brain tissues and structures, the location and shape of the ...

New oral anticoagulants for prophylaxis of stroke : Results of an expert conference on practical use in geriatric patients.
This article highlights the outcome of an expert meeting on the practical use of NOAC in elderly patients. An interdisciplinary group of experts discussed the current situation of stroke prevention in geriatric patients and its practical management in daily clinical practice. The topic was examined through focused impulse presentations and critical analyses as the basis for the expert consensus. The key issues are summarized in this paper. The European Society of Cardiology (ESC) guidelines from 2012 for the management of patients with non-valvular AF recommend NOAC as the preferred treatment and vitamin K antagonists (VKA...

Rationale and design of Short-Term EXenatide therapy in Acute ischaemic Stroke (STEXAS): a randomised, open-label, parallel-group study
Conclusions As GLP-1 analogues require elevated glucose levels to exert their insulin potentiating activity, the use of exenatide in the management of hyperglycaemia in AIS may reduce the incidence of hypoglycaemia, thereby conferring a benefit in morbidity and mortality for patients in the long term. Trial registration number ACTRN12614001189617.

Assessing and treating pain associated with stroke, multiple sclerosis, cerebral palsy, spinal cord injury and spasticity. Evidence and recommendations from the Italian Consensus conference on Pain in Neurorehabilitation.
ian Consensus Conference on Pain in Neurorehabilitation (ICCPN) Abstract Pain is a common and disabling symptom in patients with stroke, multiple sclerosis (MS), cerebral palsy (CP), spinal cord injury (SCI) and other conditions associated with spasticity, but data on its prevalence, and natural history, as well as guidelines on its assessment and treatment in the field of neurorehabilitation, are largely lacking. The Italian Consensus Conference on Pain in Neurorehabilitation (ICCPN) searched and evaluated current evidence on the frequency, evolution, predictors, assessment, and pharmacological and non-pharmacolo...

Carotid artery stenting versus no stenting assisting thrombectomy for acute ischaemic stroke: protocol for a systematic review of randomised clinical trials with meta-analyses and trial sequential analyses
DiscussionThis systematic review of carotid stenting in endovascular management of acute ischaemic stroke in patients with concomitant extracranial carotid lesions and intracranial embolism will assess benefits and harms of this intervention and assesses whether carotid stenting should be encouraged or avoided in acute ischaemic stroke and identify targets for further research.Systematic review registrationPROSPEROCRD42016033346

Towards personalised intra-arterial treatment of patients with acute ischaemic stroke: a study protocol for development and validation of a clinical decision aid
Introduction Overall, intra-arterial treatment (IAT) proved to be beneficial in patients with acute ischaemic stroke due to a proximal occlusion in the anterior circulation. However, heterogeneity in treatment benefit may be relevant for personalised clinical decision-making. Our aim is to improve selection of patients for IAT by predicting individual treatment benefit or harm. Methods and analysis We will use data collected in the Multicenter Randomized Clinical Trial of Endovascular Treatment for Acute Ischemic Stroke in the Netherlands (MR CLEAN) trial to analyse the effect of baseline characteristics on outcome and tr...

The association between human herpesvirus infections and stroke: a systematic review protocol
Introduction After primary infection, human herpesviruses establish latency and persist lifelong. Periodic virus reactivation can lead to serious inflammatory complications. Recent research suggests that herpesvirus reactivation may also be linked to acute stroke. An improved understanding of this relationship is vital to inform public health prevention strategies. We will review the evidence regarding the role of human herpesviruses in triggering stroke. Methods and analysis A systematic literature review of published and grey literature studies with a human herpesvirus (infection or reactivation) as an exposure and stro...

Perioperative Stroke Screening Tool (Phase I)
Within 12-months, two patients exhibited symptoms of stroke that were not recognized in a timely manner in the PACU at UMMC. Incidence of perioperative stroke in non-cardiac, non-neurological surgery patients is approximately 1 per 1000 cases but is associated with an eight fold increase in mortality. Due to confounding factors of anesthesia and frequent opioid administration, stroke symptoms can easily be masked. No screening tool for perioperative stroke in this population was found in the literature.

MIND diet may slow cognitive decline in stroke survivors
(Rush University Medical Center) A diet created by researchers at Rush University Medical Center may help substantially slow cognitive decline in stroke survivors, according to preliminary research presented on Jan. 25, at the American Stroke Association's International Stroke Conference 2018 in Los Angeles. The finding are significant because stroke survivors are twice as likely to develop dementia compared to the general population.

Access to acute care resources in various income settings to treat new-onset stroke: A survey of acute care providers
ConclusionThe striking finding from this study was that there was essentially very little difference between the responses between LMIC and HIC respondents with a few notable exceptions. The findings also propose a universal lack of adherence to the 2013 AHA/ASA stroke management guideline by both groups, in contrast to the good reported knowledge thereof. Carefully planned qualitative research is needed to identify the barriers to achieving the 2013 AHA/ACA recommendations.

Intraoperative Stroke During Robotic Totally Endoscopic Coronary Artery Bypass
THE INCIDENCE OF stroke after coronary artery bypass graft (CABG) surgery is between 1% and 5% and is one of the most detrimental complications.1 Off-pump CABG is associated with a lower stroke rate than traditional on-pump CABG (0.7% v 1.4%).2 Stroke after CABG is associated with increased morbidity and mortality and increased cost secondary to longer hospital stays and rehabilitation. The authors ’ presented case demonstrates the complexity of perioperative management of a patient scheduled for elective off-pump total endoscopic coronary artery bypass (TECAB) grafting with recent transient ischemic attack (TIA), stroke...
