Complex Case of Skull Base Osteomyelitis Secondary to Chronic Otitis Media
We present a case of a 58-year-old male with type II diabetes managed with metformin and insulin, who presented to the clinic with left chronic otitis media, persistent drainage, a stenotic meatus, and a prior history of 3 canal wall-down mastoidectomies and antibiotic therapy. A revision tympanoplasty with mastoidectomy was performed, and during the postoperative period, the patient had persistent pain and otorrhea, which were managed with opioids and several courses of antibiotic therapy. After symptoms persisted, imaging and culture ultimately led to the diagnosis of fungal skull base osteomyelitis, which was eventually...

Conclusions Implanted Hair Follicle Cells Produce Remodeling of Scar Tissue Assessment of Somatic Mosaicism as a Biomarker of Aging The Gut Microbiome of Centenarians https://www.fightaging.org/archives/2023/01/the-gut-microbiome-of-centenarians/ The state of the gut microbiome is arguably as influential on health as exercise. Various microbial species present in the gut produce beneficial metabolites, such as butyrate, or harmful metabolites, such as isoamylamine, or can provoke chronic inflammation in a variety of ways. An individual can have a better or worse microbiome, assessing these and other fu...

Low-Carbohydrate High-Fat Diet: A SWOC Analysis
Metabolites. 2022 Nov 17;12(11):1126. doi: 10.3390/metabo12111126.ABSTRACTInsulin resistance (IR) plays a role in the pathogenesis of many diseases, such as type 2 diabetes mellitus, cardiovascular disease, non-alcoholic fatty liver disease, obesity, and neurodegenerative diseases, including Alzheimer's disease. The ketogenic diet (KD) is a low-carbohydrate/high-fat diet that arose in the 1920s as an effective treatment for seizure control. Since then, the KD has been studied as a therapeutic approach for various IR-related disorders with successful results. To date, the use of the KD is still debatable regarding its safet...

Arch Acad Emerg Med. 2022 Aug 21;10(1):e68. doi: 10.22037/aaem.v10i1.1709. eCollection 2022.ABSTRACTHamman's syndrome is an uncommon clinical entity characterized by an idiopathic spontaneous pneumomediastinum as a result of a sudden increase in intra-alveolar pressure. Herein, we report a case of an 18-year-old man complaining of chest discomfort and progressive weight loss, ultimately diagnosed with ...

Progress in oral insulin delivery by PLGA nanoparticles for the management of diabetes
Drug Discov Today. 2022 Oct 5:103393. doi: 10.1016/j.drudis.2022.103393. Online ahead of print.ABSTRACTCurrently, the only practical way to treat type 1 and advanced insulin-dependent type 2 diabetes mellitus (T1/2DM) is the frequent subcutaneous injection of insulin, which is significantly different physiologically from endogenous insulin secretion from pancreatic islets and can lead to hyperinsulinemia, pain, and infection in patients with poor compliance. Hence, oral insulin delivery has been actively pursued to revolutionize the treatment of insulin-dependent diabetes. In this review, we provide an overview of recent p...
