Efficacy of Lower Doses of Pioglitazone after Stroke or Transient Ischemic Attack In Patients with Insulin Resistance
CONCLUSIONS: Lower doses of pioglitazone appear to confer much of the benefit with less adverse effects than the full dose. Further study is needed to confirm these findings so that clinicians may optimize dosing of this secondary prevention strategy in stroke patients. This article is protected by copyright. Diabetes and Ischemic Stroke: An Old and New Relationship an Overview of the Close Interaction between These Diseases
Int J Mol Sci. 2022 Feb 21;23(4):2397. doi: 10.3390/ijms23042397.ABSTRACTDiabetes mellitus is a comprehensive expression to identify a condition of chronic hyperglycemia whose causes derive from different metabolic disorders characterized by altered insulin secretion or faulty insulin effect on its targets or often both mechanisms. Diabetes and atherosclerosis are, from the point of view of cardio- and cerebrovascular risk, two complementary diseases. Beyond shared aspects such as inflammation and oxidative stress, there are multiple molecular mechanisms by which they feed off each other: chronic hyperglycemia and advanced...

Ischemic risk in patients with heart failure with preserved ejection fraction: A post hoc analysis of the TOPCAT data
CONCLUSIONS: HFpEF patients from Americas were at a high risk of ischemic events, which was associated with mortality risk. A subset of baseline characteristics and HF hospitalization during follow-up could predict ischemic event.PMID:35101783 | DOI:10.1016/j.atherosclerosis.2022.01.013

Short- and Long-term cardiovascular outcomes in insulin-treated versus non-insulin-treated diabetes mellitus patients after percutaneous coronary intervention: a systematic review and meta-analysis
CONCLUSION: Despite a tremendous technical success rate of multi-vessel stenting, people living with diabetes who were being treated with insulin had higher long-term, and short-term mortality rates, MI, TLR, TVR, and stroke compared to people living with diabetes who were being treated with means other than insulin and are more prone to detrimental cardiovascular outcomes.PMID:34906538 | DOI:10.1016/j.ihj.2021.12.004

Endothelial Cell CD36 Reduces Atherosclerosis and Controls Systemic Metabolism
Front Cardiovasc Med. 2021 Nov 23;8:768481. doi: 10.3389/fcvm.2021.768481. eCollection 2021.ABSTRACTHigh-fat Western diets contribute to tissue dysregulation of fatty acid and glucose intake, resulting in obesity and insulin resistance and their sequelae, including atherosclerosis. New therapies are desperately needed to interrupt this epidemic. The significant idea driving this research is that the understudied regulation of fatty acid entry into tissues at the endothelial cell (EC) interface can provide novel therapeutic targets that will greatly modify health outcomes and advance health-related knowledge. Dysfunctional ...

Estimated glucose disposal rate and risk of stroke and mortality in type 2 diabetes: a nationwide cohort study
Insulin resistance contributes to the development of type 2 diabetes (T2D) and is also a cardiovascular risk factor. The aim of this study was to investigate the potential association between insulin resistanc...

Ten-year all-cause death following percutaneous or surgical revascularization in patients with prior cerebrovascular disease: insights from the SYNTAX Extended Survival study
ConclusionPrior CEVD was associated with a significantly increased risk of 10  year all-cause death which was similar in patients treated with PCI or CABG. These results do not support preferential referral for PCI rather than CABG in patients with prior CEVD.Trial registration: SYNTAX: ClinicalTrials.gov reference: NCT00114972. Triglyceride-glucose index and the risk of stroke and its subtypes in the general population: an 11-year follow-up
Triglyceride-glucose (TyG) index was recently suggested to be a reliable surrogate marker of insulin resistance. We aim to investigate the associations between baseline and long-term TyG index with subsequent ...
