Association of Dental Infections with Intracranial Atherosclerotic Stenosis
CONCLUSION: We report significant associations between mild-moderate tooth loss, severe tooth loss and severe PD with <50% ICAS as well as an association between gingivitis and ≥50% ICAS. We did not find an association between dental caries and ICAS.PMID:37121226 | DOI:10.1159/000530829

Rationale and design of a randomized trial of apixaban vs warfarin to evaluate atherosclerotic calcification and vulnerable plaque progression
This study is a single‐center, prospective, randomized, open‐label study. From May 2014 to December 2015, 66 patients with nonvalvular AF who experienced VKA therapy were enrolled. Patients were randomized into either warfarin or apixaban cohorts and followed for 52 weeks. The primary objective is to compare the rate of change in coronary artery calcification (CAC) from baseline to follow‐up in apixaban vs warfarin cohorts. The key secondary objective is to compare the rate of incident plaques and quantitative changes in plaque types between patients randomized to either warfarin or apixaban cohorts using serial coro...

Cardiac CT angiography for device surveillance after endovascular left atrial appendage closure
Conclusion CCTA appears to be a feasible alternative to transoesophageal echocardiography for post-LAA device surveillance to evaluate for device thrombus, residual leak, embolization, position, and pericardial effusion.

Safety and effectiveness of compassionate use of LARIAT® device for epicardial ligation of anatomically complex left atrial appendages
Conclusions Despite a high preclusion rate, percutaneous LAA ligation may be safely and effectively performed on compassionate grounds using the first-generation LARIAT® device with surgical standby in patients with large and complex LAA.

Antithrombotic therapy for atrial fibrillation and coronary artery disease in older patients
Conclusions Among older patients with AF and CAD, overall warfarin use was low. Patients ≥80 years old at highest stroke risk received warfarin in similar proportions to the overall cohort. Further investigation into optimizing antithrombotic strategies in this population is warranted.

Simultaneous onset of myocardial infarction and ischemic stroke in a patient with atrial fibrillation: Multiple territory injury revealed on angiography and magnetic resonance
An 84-year-old man with a history of hypertension and paroxysmal atrial fibrillation (AF) who received no anticoagulant drugs experienced acute chest pain and transient loss of consciousness. He was transferred to our emergency room. His initial electrocardiogram showed sinus rhythm with ST-segment elevation in the I, aVL, and V1–V6 leads. His blood pressure was 158/92mmHg and his pulse was regular at 70beats per minute. A chest radiograph showed increased heart size and pulmonary vascular congestion. His troponin T level was elevated (more than 0.1ng/mL). He was diagnosed with ST-elevation myocardial infarction (MI) and...

Imaging the aortic arch to improve diagnostic and prognostic accuracy in transient ischemic attack patients
This study raises a final relevant question: since the prognosis of TIA patients worsens in proportion to the severity of AAA, what therapeutic options are available for these patients? The clinical guidelines recommend risk factor modification and therapy with statins (Class IIa, level of evidence C), an approach consistent with treatment of any patient affected by atherosclerosis. Treatment with coumadin or anti-platelet agents can be considered in patients with AAA >4 mm, i.e. severe AAA, but it remains very controversial because of a potential risk of thrombo-embolism (Class IIb, level of evidence C) . Finally, surgi...
