Impact of organizational innovation in stroke care on post-acute phase health expenses in France
ConclusionsPost-stroke expenditure, the first year after stroke is mainly made up of rehabilitation expenses and depends on the type and intensity of the rehabilitation. Concerning inital care in neurovascular unit, it allows for better quality of life.Key messagesPost-stroke expenditure, the first year after stroke is mainly made up of rehabilitation expenses and depends on the type and intensity of the rehabilitation.Inital care in neurovascular unit, it allows for better quality of life.

The effects of the COVID-19 lockdown and socio-economic factors on stroke hospitalizations in France
ConclusionsDuring the first national lockdown, there has been an overall decrease in stroke admission rates. Socio-economic determinants such as low-skilled jobs were independently associated with an increase in the stroke admission incidence rate ratio, while we did not find any independent effect from the local COVID 19 burden and hospital capacities.Key messagesThe first national lockdown led to an overall decrease in stroke admissions in France. This decrease varied between counties according to socio-economic determinants.

Access to stroke care in Europe: results of a vignette approach
ConclusionsWhile stroke related services are formally covered in almost all countries, considerable access problems exist in several countries. Future cross-country surveys of access to stroke care should collect quantitative data related to proportions of patients receiving particular services as well as qualitative data on access barriers.

Effects of prismatic adaptation on balance and postural disorders in patients with chronic right stroke: protocol for a multicentre double-blind randomised sham-controlled trial
The objective is to investigate the effects of this intervention on balance but also on postural disorders, subjective straight-ahead, longitudinal body axis and autonomy in patients with chronic right stroke lesion. Methods and analysis In this multicentre randomised double-blind sham-controlled trial, we will include 28 patients aged from 18 to 80 years, with a first right supratentorial stroke lesion at chronic stage (≥12 months) and having a bearing ≥60% of body weight on the right lower limb. Participants will be randomly assigned to the experimental group (performing pointing tasks while wearing glasses shifti...

Association between reported work in cold environments and stroke occurrence in the CONSTANCES cohort: a prospective study
Conclusions This study did not reveal a significant excess risk of stroke for occupational exposures to low temperatures. Further studies are needed to better assess the effect of preventive measures and very low temperature on occurrence of cardiovascular diseases.

Effects of healthcare system transformations spurred by the COVID-19 pandemic on management of stroke and STEMI: a registry-based cohort study in France
Conclusions The pandemic did not markedly alter the functioning of the emergency network. Although stroke patient management deteriorated, the resilience of the STEMI pathway was linked to its stronger structuring. Transversal reorganisations, aiming at concentrating resources on emergency care, contributed to maintenance of the quality of care. Trial registration number NCT04979208.

Identifying patients with cerebral infarction within the time window compatible with reperfusion therapy, diagnostic performance of glutathione S-transferase-{pi} (GST-{pi}) and peroxiredoxin 1 (PRDX1): exploratory prospective multicentre study FLAG-1 protocol
Introduction Plasma biomarkers may be useful in diagnosing acute cerebral infarction requiring urgent reperfusion, but their performance remains to be confirmed. If confirmed, these molecules could be used to develop rapid and reliable decentralised measurement methods, making it possible to initiate reperfusion therapy before hospital admission. The FLAG-1 large prospective study will constitute a plasma bank to assess the diagnostic performance of two biomarkers: glutathione S-transferase- and peroxiredoxin 1. These molecules are involved in the oxidative stress response and could identify cerebral infarction within a th...

Platelet transmission electron microscopy for the assessment of poor biological response to antiplatelet agent: pilot descriptive and prospective study - ELECTROSTROKE
Introduction Ischaemic stroke is the leading cause of adult disability. Thus, a strategy based on an efficient antiplatelet therapy has been developed. The monitoring of antiplatelet therapy is now limited to high risk and poor prognosis patients. Indeed, the biological monitoring of the antiplatelet therapy with available platelet function assays do not provide a global integrative approach. Platelet transmission electron microscopy, recently validated for assessing distinct ultrastructural abnormalities is a reliable morphological platelet structural analysis tool which could be used to collect all the ultrastructural pl...

Distinct cardiovascular and cancer burdens associated with social position, work environment and unemployment: a cross-sectional and retrospective study in a large population-based French cohort
Conclusions Social position, work environment and unemployment are associated with distinct cardiovascular and cancerous diseases that could add up during lifetime, they should therefore be considered all together in any preventive strategy.

Smoking and blood pressure: A complex relationship.
Abstract Hypertension and tobacco smoking are two major modifiable risk factors for atheromatous disease and its cardiovascular complications. If systolic hypertension (SBP≥140mmHg and DBP<90mmHg) is the leading risk factor for stroke, smoking (nicotine) has a more powerful impact on coronary events, aortic aneurysms, and peripheral artery disease. Smoking can transiently modify the regulation of blood pressure (BP) by a swift effect on the autonomic nervous system. It also accelerates arterial aging, which plays a role in chronic hypertension. Chronic sympathetic activation induced by tobacco smoking also ha...

Hypertension in the elderly.
Abstract After 80 years old, antihypertensive treatment significantly reduces cardiovascular events. In the elderly, blood pressure target depends on patients' frailty. After 80 years, French guidelines propose to aim a SBP<150 mmHg without orthostatic hypotension and without exceeding the prescription of more than three antihypertensive drugs. The target may be more ambitious for robust elderly patients. The new 2018 European guidelines set: a stricter target for robust elderly patient aged 80 years or older (SBP between 130 and 139 mmHg and DBP between 70 and 79 mmHg); this objective is less strict for frail ...
