Validity of early MRI structural damage end points and potential impact on clinical trial design in rheumatoid arthritis
Conclusions Early MRI erosion progression is a valid measure of structural damage that could substantially decrease sample size and study duration if used as structural damage end point in RA clinical trials.

Flare in axial spondyloarthritis. The dark side of the outcome
Spondyloarthritis (SpA) is a chronic inflammatory rheumatic disease with many phenotypes,1 but the frame of the disease is still a matter of debate, particularly regarding the non-radiographic forms of axial SpA.2 3 The disease evolution may have several profiles, mainly related to the treatment strategy, balancing from periods of remission or low disease activity to flares of the disease. The recommended treatment strategies are supposed to be tailored to the disease activity, aiming to reach remission or low disease activity in a T2T strategy,4 with management of remission (reduction of dosage or increase in interval of ...

Targeting ultrasound remission in early rheumatoid arthritis: the results of the TaSER study, a randomised clinical trial
Conclusions In early RA, a MSUS-driven T2T strategy led to more intensive treatment, but was not associated with significantly better clinical or imaging outcomes than a DAS28-driven strategy. Trial registration number NCT00920478.

Cow's Milk Allergy and Bone Mineral Density in Prepubertal Children
CONCLUSIONS: These prepubertal children with persistent CMA had lower lumbar spine BMD z scores than children with NCMA, which likely resulted from lower calcium intake.

Radiographic characterization of the retroantral ethmoid cell
ConclusionThis study demonstrates the relatively high prevalence of the RAEC in our patient population. The majority of RAECs showed both evidence of disease and that they were not completely opened during previous surgery. Recognition of this anatomic entity may allow for more thorough ethmoidectomy.

Ultrasound-detected inflammation predicts radiographic progression in hand osteoarthritis after 5 years
Conclusions Ultrasound-detected GS synovitis and PD signals were significantly associated with radiographic progression after 5 years. This study supports the use of ultrasound as a tool to detect patients with hand OA who are likely to progress.

Gliomatosis, to Be or Not to Cerebri? (P6.299)
Conclusions: The clinical challenge to treat patients with extensive, diffuse infiltration of molecularly-diverse gliomas is as yet unmet. Uncovering the drivers of this condition may come with a deeper understanding of patient genetic and tumor microenvironmental predispositions toward the diffuse GC infiltrating pattern.Disclosure: Dr. Gatson has nothing to disclose. Increasing synovitis and bone marrow lesions are associated with incident joint tenderness in hand osteoarthritis
Conclusions The Oslo hand OA cohort is the first study with longitudinal hand MRIs. Increasing synovitis and BMLs were significantly associated with incident joint tenderness, whereas no significant associations were found for decreasing or loss of synovitis and BMLs.

QUESTION 1: Should we be using bisphosphonates for osteonecrosis complicating childhood acute lymphoblastic leukaemia?
Scenario A 15-year-old girl with acute lymphoblastic leukaemia (ALL) who has been on high-dose steroids develops a painful right hip. Although plain radiographs are normal, there are changes indicating osteonecrosis on MRI. You remember that osteonecrosis can develop after use of high-dose steroids, and recall a discussion with an orthopaedic surgeon who had said he thought bisphosphonates could help in some patients. You wonder whether they would be of value for your patient. Structured clinical question In a patient with osteonecrosis, presumed secondary to steroid use, in childhood ALL (patient), what is the evidence fo...

Early introduction of food to prevent food allergy. The LEAP study (Learning Early about Peanut)
Objective: To report a case of a preschool girl who developed acute urinary retention associated with constipation. Case description: A girl aged six years old presented a 24 h history of inability to urinate. She was went twice to the emergency room during this period. In the first admission, 12 h after the onset of the symptoms, she presented abdominal pain and acute urinary retention. After the drainage by urinary catheterization of 300 mL of clear urine, she presented relief of the symptoms and, as urinalysis had no change, the patient was discharged home. Twelve hours after the first visit, she returned to the emergen...

Serum Jo-1 Autoantibody and Isolated Arthritis in the Antisynthetase Syndrome: Review of the Literature and Report of the Experience of AENEAS Collaborative Group
Abstract Anti-Jo-1 is the most frequently detectable antibody in the antisynthetase syndrome (ASSD), an autoimmune disease characterized by the occurrence of arthritis, myositis, and interstitial lung disease (ILD). Recently, we organized an international collaborative group called American and European NEtwork of Antisynthetase Syndrome (AENEAS) for the study of this rare and fascinating disease. The group collected and published one of the largest series of ASSD patients ever described and with one of the longer follow-up ever reported. The number of participating centers is steadily increasing, as well as the a...

Changes in the anticitrullinated peptide antibody response in relation to therapeutic outcome in early rheumatoid arthritis: results from the SWEFOT trial
Conclusions The influence of early antirheumatic therapy on ACPA seroreversions was markedly different across specificities, and early disappearance of anti-cVim antibodies associated with better radiological outcome. Thus, these data suggest that the disappearance of particular ACPA reactivities may be beneficial in early RA. Trial registration number WHO database at the Karolinska institute: CT20080004; and clinicaltrials.gov: NCT00764725.
