Medical adjuvant treatment to increase patency of arteriovenous fistulae and grafts
CONCLUSIONS: The meta-analyses of three studies for ticlopidine (an antiplatelet treatment), which all used the same dose of treatment but with a short follow-up of only one month, suggest ticlopidine may have a beneficial effect as an adjuvant treatment to increase the patency of AVFs and AVGs in the short term. There was insufficient evidence to determine if there was a difference in graft patency between placebo and other treatments such as aspirin, fish oil, clopidogrel, dipyridamole, dipyridamole plus aspirin, warfarin, sulphinpyrazone and GTN patch. The certainty of the evidence was low to moderate due to short follo...

Stakeholder Perspectives on Factors Related to Deprescribing Potentially Inappropriate Medications in Older Adults Receiving Dialysis
CONCLUSIONS: Challenges to integration of deprescribing into dialysis clinics included siloed health systems, time constraints, co-management behaviors, and clinician and patient knowledge and attitudes towards deprescribing.PMID:37499693 | DOI:10.2215/CJN.0000000000000229

The pathogenesis of hemodialysis vascular access failure and systemic therapies for its prevention: Optimism unfulfilled
Abstract In patients receiving hemodialysis, the provision of safe and effective vascular access using an arteriovenous fistula or graft is regarded as a critical priority by patients and health professionals. Vascular access failure is associated with morbidity and mortality, such that strategies to prevent these outcomes are essential. Inadequate vascular remodeling and neointimal hyperplasia resulting in stenosis and frequently thrombosis are critical to the pathogenesis of access failure. Systemic medical therapies with pleiotropic effects including antiplatelet agents, omega‐3 polyunsaturated fatty acids (fish oils)...
