Efficacy and Safety of Upacicalcet in Hemodialysis Patients with Secondary Hyperparathyroidism: A Randomized Placebo-Controlled Trial
CONCLUSIONS: Upacicalcet, a novel injectable calcimimetic, is effective and safe for secondary hyperparathyroidism patients receiving HD.CLINICAL TRIAL REGISTRY NAME AND REGISTRATION NUMBER: Phase 3 Study of SK-1403, NCT03801980.PMID:37696667 | DOI:10.2215/CJN.0000000000000253

Coupling of renal sodium and calcium transport: A modeling analysis of transporter inhibition and sex differences
Am J Physiol Renal Physiol. 2023 Aug 24. doi: 10.1152/ajprenal.00145.2023. Online ahead of print.ABSTRACTCalcium (Ca2+) transport along the nephron occurs via specific transcellular and paracellular pathways, and is coupled to the transport of other electrolytes. Notably sodium (Na+) transport establishes an electrochemical gradient to drive Ca2+ reabsorption. Hence, alterations in renal Na+ handling, under pathophysiological conditions or pharmacological manipulations, can have major effects on Ca2+ transport. An important class of pharmacological agent is diuretics, which are commonly prescribed for the management of blo...

Sodium zirconium cyclosilicate reconciles management of hyperkalemia and continuity of renin-angiotensin-aldosterone system inhibitors: a retrospective observational study
CONCLUSIONS: Our study suggests that administering sodium zirconium cyclosilicate to patients with hyperkalemia allows for a higher continuation rate of angiotensin-converting enzyme inhibitors/angiotensin receptor blockers than calcium polystyrene sulfonate. These findings suggest that sodium zirconium cyclosilicate has potential benefits for patients with chronic kidney disease receiving renin-angiotensin-aldosterone system inhibitors.PMID:37608241 | DOI:10.1007/s40620-023-01743-4

Renal sarcoidosis presenting with chronic kidney disease and hypercalcemia
We describe a 72-year-old male who presented with chronic kidney disease and elevated serum calcium and angiotensin-converting enzyme. Renal biopsy pathology showed chronic granulomatous interstitial nephritis. Renal function was significantly improved after glucocorticoid therapy. This case emphasizes that chronic kidney disease and hypercalcemia can be clues for renal sarcoidosis. Early renal biopsy and projective treatment is beneficial for renal outcome.PMID:37548467 | DOI:10.5414/CN111115

Blood pressure management and long-term outcomes in kidney transplantation: a holistic view over a 35-year period
CONCLUSIONS: Pharmacological recommendations and sodium intake reduction are poorly respected, but even when respected, do not result in better blood pressure control, or patient or graft survival. Uncontrolled blood pressure, not the use of specific antihypertensive classes, is associated with reduced patient, and to a lesser extent, reduced graft survival, even using the 120/80 mmHg cut-off.PMID:37548826 | DOI:10.1007/s40620-023-01706-9
