A Clinical Case of Electronic Health Record Drug Alert Fatigue: Consequences for Patient Outcome
Despite advances in electronic medication order entry systems, it has been well established that clinicians override many drug allergy alerts generated by the electronic health record. The direct clinical consequences of overalerting clinicians in a pediatric setting have not been well demonstrated in the literature. We observed a patient in the PICU who experienced complications as a result of an extended series of non–evidence-based alerts in the electronic health record. Subsequently, evidence-based allergy alerting changes were made to the hospital’s system. Incorporating clinical evidence in electronic dru...

Abstract 004: Electronic Health Record-Based Overestimation of Failure to Prescribe Statins in Patients With Coronary Artery Disease in a Large, Academic, Multispecialty Clinic Practice Session Title: Abstract Oral Session: QCOR 2017 Young Investigator Award Finalists
Conclusions: Among outpatients with presumed CAD who were identified by EHR data query as "failures" to receive statin prescriptions, an estimated fewer than one-third constituted a true gap in care. These findings raise concern about the accuracy of EHR data query in measuring performance in the Meaningful Use era.

A value set for documenting adverse reactions in electronic health records
ConclusionThis work contributes a value set, harmonized with existing data, to improve the consistency and accuracy of reaction documentation in electronic health records, providing the necessary building blocks for more intelligent clinical decision support for allergies and adverse reactions.

Current Epidemiology and Management of Radiocontrast-Associated Acute- and Delayed-Onset Hypersensitivity: A Review of the Literature.
Authors: Macy EM Abstract Radiocontrast-associated acute-onset hypersensitivity reactions now occur less frequently than before 1990, when high-osmolar, ionic, radiocontrast agents were widely used. Premedication with corticosteroids and antihistamines does not reliably prevent recurrent low-osmolar radiocontrast-associated acute hypersensitivity reactions. Corticosteroid prophylaxis for acute hypersensitivity currently causes more morbidity than benefit. The specific radiocontrast agent that is associated with a patient's adverse reaction must be displayed in the drug intolerance or drug "allergy" field of their e...

Cough Presentation and Cough-Related Healthcare Utilization in Tertiary Care: Analysis of Routinely Collected Academic Institutional Database
ConclusionsCough is a common chief complaint at allergy and asthma clinics, but the clinical presentation may be heterogeneous. Further studies are needed to understand long-term outcomes and reduce the disease burden.

Electronic health records boost efficiency, safety
“Question: Why are electronic health record systems important? Answer: EHR systems allow health care providers and their staff to more easily extract the information they need about patients. EHR systems improve patient safety by checking drug-drug interactions, drug-allergy interactions, and also allow for remote review of select patient information. Q: How will EHR [...]

The Editors' Choice
Chronic rhinosinusitis (CRS) is a common inflammatory condition of the paranasal sinuses with an unclear cause. The factors and illnesses associated with the development of CRS have not been examined longitudinally. In this issue Tan et al (p 1350) study the epidemiology of CRS using a case-control study on incident CRS cases. By using 10 years of electronic health records from the Geisinger Clinic, which serves a primary care population of a geographically large and demographically stable population in central Pennsylvania, the authors were able to examine patterns of diagnoses and health resource use preceding a new di...

Incidence and associated premorbid diagnoses of patients with chronic rhinosinusitis
Background: Chronic rhinosinusitis (CRS) is a prevalent condition with underexplored risk factors. Objectives: We sought to determine CRS incidence and evaluate associations with a range of premorbid medical conditions for chronic rhinosinusitis without nasal polyps (CRSsNP) and chronic rhinosinusitis with nasal polyps (CRSwNP) using real-world clinical practice data. Methods: Electronic health records data from 446,480 Geisinger Clinic primary care patients were used for a retrospective longitudinal cohort study for data from 2001-2010. By using logistic regression, newly diagnosed CRS cases between 2007 and 2009 were compa...

Do provider attitudes about electronic health records predict future electronic health record use?
Discussion Comfort with computers and attitudes about EHRs did not predict future use of the EHR functions. Our findings suggest that meaningful use of the EHR may not be affected by providers׳ prior attitudes about EHRs.
