Protective effects of corticosteroids in coronary artery bypass graft surgery alone or combined with valvular surgery: an updated and comprehensive meta-analysis and systematic review
This systematic review with meta-analysis sought to determine the protective effects of corticosteroids on clinical outcomes following coronary artery bypass grafting (CABG). Medline, Embase, Elsevier and Sciences online database as well as Google scholar literature were used for selecting appropriate studies with randomized controlled design. The effect sizes measured were odds ratio (OR) for categorical variables and weighted mean difference with 95% confidence interval (CI) for calculating differences between mean values of duration of hospitalization in intervention and control groups. Values of P < 0.1 for Q-test o...

Bypass surgery an “uncommon” cause of memory loss, cognitive decline
Coronary artery bypass surgery (CABG) offers a new lease on life for thousands of people each year whose hearts aren’t getting the blood they need to work properly. But it has also been blamed for “brain fog,” a loss of memory and thinking skills that follows the procedure in some people. Such brain problems are often called cognitive impairment. The operation itself may not be to blame, according to a review in today’s Annals of Internal Medicine. For the review, a team of researchers—mostly from the U. S. Department of Veteran’s Affairs—synthesized data from 17 clinical trials and four w...

New-onset atrial fibrillation at discharge in patients after coronary artery bypass surgery: short- and long-term morbidity and mortality ADULT CARDIAC
CONCLUSIONS A majority of NOAF patients revert to SR during the first months after surgery. On the other hand, 20–25% of NOAF patients develop chronic AF during long-term follow-up. Almost half of the NOAF patients were hospitalized during follow-up due to cardiovascular causes. Similarly, NOAF was associated with increased mortality due to cardiac causes, but not increased risk of stroke. This highlights the need for proper oral anticoagulation therapy in these patients.

Safety and efficacy of glucose-insulin-potassium treatment in coronary artery bypass graft surgery and percutaneous coronary intervention
The purpose of this meta-analysis was to evaluate protective effects of glucose–insulin–potassium (GIK) on outcomes after coronary artery bypass grafting (CABG) or percutaneous coronary intervention (PCI). We systematically searched Medline/Pubmed, Elsevier, Embase, Web of Knowledge and Google Scholar. A total of 1206 studies were retrieved during the extensive literature search of all major databases; however, 38 trials reporting the end-point of interest were selected. We performed a pooled analysis of outcomes following PCI: incidence of cardiac arrest [odds ratio (OR) of 0.91; 95% confidence interval (CI): ...

Increased IL-17A in atrial fibrillation correlates with neutrophil to lymphocyte ratio.
CONCLUSIONS: The increase in IL-17A levels is related to the AF pathology mediated by neutrophils and monocytes. The current study signifies the role of immune cells and cytokines in the pathology of AF. PMID: 25549592 [PubMed - indexed for MEDLINE]

Topical amiodarone to prevent postoperative atrial fibrillation: Need for further study
Postoperative atrial fibrillation (AF) is a common complication after cardiac surgery that increases hospital stay and the risk of stroke.1 Prophylactic systemic administration of amiodarone in the perioperative period prevents postoperative AF2; however, its challenging side-effect profile limits its routine and widespread use as prophylaxis.3 It was therefore with great interest that we recently read the report by Feng and colleagues4 describing their success in applying a topical amiodarone-releasing hydrogel on the atrial surface during coronary artery bypass grafting operations.

Coronary artery bypass surgery without cardioplegia: hospital results in 8515 patients ADULT CARDIAC
CONCLUSIONS Isolated CABG with CPB using non-cardioplegic methods proved very safe, with low mortality and morbidity. These methods are simple and expeditious and remain as very useful alternative techniques of myocardial preservation.

Abstract 138: Contemporary Patterns of Use of Antiplatelet Agents in Patients With Acute Myocardial Infarction: Insight From the National Cardiovascular Data Registry (NCDR) Session Title: Abstract Poster Session I and Reception
Conclusions: Our contemporary dataset shows a modest but significant increase in use of ticagrelor early and at discharge, with simultaneous decline in the use of clopidogrel and prasugrel in patients presenting with AMI.

Propensity Matched Analysis of del Nido Cardioplegia in Adult Coronary Artery Bypass Grafting: Initial Experience With 100 Consecutive Patients
Conclusions In our initial experience, DC provided equivalent myocardial protection and clinical outcomes to BC in adult isolated CABG patients. DC was associated with lower cardiopulmonary bypass glucose levels than BC and demonstrated the feasibility of single-dose administration for routine coronary operations.

Effect of Posterior Pericardiotomy on the Incidence of Postoperative Atrial Fibrillation in 1000 Consecutive Isolated CABG
Purpose: Postoperative atrial fibrillation (POAF) is the most common arrhythmia and morbidity after cardiac surgery and is associated with prolonged hospital stay, stroke and increased costs. At the Royal Hobart Hospital, one of the two surgeons routinely performed posterior pericardiotomy for patients undergoing coronary artery bypass grafting (CABG). We took this opportunity to review risk factors associated with postoperative atrial fibrillation and in particular if posterior pericardiotomy reduces the incidence of postoperative atrial fibrillation.

Comparison of isoflurane and sevoflurane in cardiac surgery: a randomized non-inferiority comparative effectiveness trial.
CONCLUSION: Sevoflurane is non-inferior to isoflurane on a composite outcome of prolonged ICU stay and all-cause 30-day mortality. Sevoflurane is not superior to isoflurane on any other of the clinically important outcomes. This trial was registered at clinicaltrials.gov; NCT01477151. Management of New ‐Onset Postoperative Atrial Fibrillation Utilizing Insertable Cardiac Monitor Technology to Observe Recurrence of AF
Conclusion: POAF may represent a propensity for recurrent PAF, and not simply a transient consequence of post‐operative stress and inflammation. Better detection of recurrent AF might identify patients at risk for stroke who would benefit from continuing anticoagulation. This article is protected by copyright. All rights reserved
