Direct thrombectomy versus bridging thrombolysis with mechanical thrombectomy in middle cerebral artery stroke: a real-world analysis through National Inpatient Sample data
CONCLUSIONS: In the analysis of the NIS database, the MT+IVT group had significantly higher rates of minimal disability at the time of hospital discharge versus the MT-alone group, despite a higher rate of ICH. The question of whether to treat patients with MT+IVT rather than MT alone is currently being addressed in ongoing prospective clinical trials (SWIFT-DIRECT [NCT03494920], MR CLEAN-NO IV [ISRCTN80619088], and DIRECT-SAFE [NCT03494920]). The results of these studies will contribute to greater understanding and progressive improvement in outcomes for AIS patients.PMID:34198246 | DOI:10.3171/2021.4.FOCUS21132

E-025 Aspiration thrombectomy of acute ischemic stroke patients with intracranial atherosclerotic disease or tandem lesion
ConclusionAspiration thrombectomy was effective in restoring blood flow and resulting in good 90-day functional outcome in AIS patients with anterior circulation tandem lesion, and the 90-day mortality rate in these patients was low. Early versus late start of direct oral anticoagulants after acute ischaemic stroke linked to atrial fibrillation: an observational study and individual patient data pooled analysis
Conclusions Our results do not corroborate concerns that an early DOAC-start might excessively increase the risk of ICH. The sevenfold higher risk of recurrent AIS than ICH suggests that an early DOAC-start might be reasonable, supporting enrolment into randomised trials comparing an early versus late DOAC-start.

Conjugate upward gaze paralysis with unilateral ptosis caused by a unilateral midbrain infarction
A 73-year-old woman with atrial fibrillation presented with a sudden right hemiparesis, with diplopia and left ptosis, and was admitted at an Emergency Unit. The neurological examination found fluctuations on consciousness level, predominant crural right hemiparesis and right central facial paralysis without sensitive abnormalities.
