Single-center experience of 105-minimalistc transfemoral transcatheter aortic valve replacement and its outcome
CONCLUSION: Minimalist, conscious sedation, transfemoral transcatheter aortic valve replacement when done following a standard protocol is safe and effective.PMID:34154746 | PMC:PMC8322745 | DOI:10.1016/j.ihj.2021.01.023

Evaluation and Management of Aortic Stenosis in Chronic Kidney Disease: A Scientific Statement From the American Heart Association
Circulation. 2021 May 13:CIR0000000000000979. doi: 10.1161/CIR.0000000000000979. Online ahead of print.ABSTRACTAortic stenosis with concomitant chronic kidney disease (CKD) represents a clinical challenge. Aortic stenosis is more prevalent and progresses more rapidly and unpredictably in CKD, and the presence of CKD is associated with worse short-term and long-term outcomes after aortic valve replacement. Because patients with advanced CKD and end-stage kidney disease have been excluded from randomized trials, clinicians need to make complex management decisions in this population that are based on retrospective and observ...

A Composite Metric for Benchmarking Site Performance in TAVR: Results from the STS/ACC TVT Registry
Conclusions: There are substantial variations in the quality of TAVR care received in the United States, and 11% of sites were identified as providing care below the average level of performance. Further study is necessary to determine structural, process-related, and technical factors associated with high- and low-performing sites.PMID:33947202 | DOI:10.1161/CIRCULATIONAHA.120.051456

Prognosis of paradoxical low-flow low-gradient aortic stenosis after transcatheter aortic valve replacement
Aims In paradoxical low-flow low-gradient severe aortic stenosis (PLFLG AS) patients, stroke volume index (SVI) is reduced despite preserved left ventricular ejection fraction (LVEF). Although reduced SVI is already known as a poor prognostic predictor, the outcomes of PLFLG AS patients after transcatheter aortic valve replacement (TAVR) have not been clearly defined. We retrospectively investigated the post-TAVR outcomes of PLFLG AS patients in comparison with normal-flow high-gradient aortic stenosis (NFHG AS) patients. Methods The current observational study included 245 patients with NFHG AS (mean transaorti...

Simplifying the approach to classical low-flow low-gradient severe aortic stenosis: A renewed emphasis on the resting transthoracic echocardiogram
Aortic stenosis (AS) represents the most commonly encountered valvular heart disease. Traditionally, severe AS is defined by an aortic valve area (AVA) less than or equal to 1.0  cm2 and mean aortic valve gradient (MG) greater than or equal to 40 mm Hg. In roughly 40% of encountered cases, however, Doppler echocardiography yields discordant findings—a calculated AVA suggestive of severe AS but a MG consistent with less-than-severe disease [1,2]. Assuming no error in measurement, these cases stem, in part, from low-flow states (defined as a resting stroke volume index, SVI, less than or equal to 35 mL/m2) and cons...
