Is Acute Carotid Artery Stent Thrombosis an Avoidable Complication?
The most serious complication of carotid artery stenting (CAS) is acute carotid artery stent thrombosis (ACAST). ACAST is a very rare complication, but it may lead to dramatic and catastrophic consequences. The most important cause is inadequate or ineffective antiaggregant therapy. It is very important to identify, before CAS, those patients who might be candidates for ACAST and to start antiplatelet therapy for them. Testing patients who are candidates for CAS for acetylsalicylic acid and clopidogrel resistance may prevent this complication.

Low-molecular-weight heparin or dual antiplatelet therapy is more effective than aspirin alone in preventing early neurological deterioration and improving the 6-month outcome in ischemic stroke patients.
CONCLUSIONS: LMWH or DAT may be more effective than aspirin alone for reducing the incidence of END and ERIS within 14 days, and is associated with improved outcomes in elderly patients and those with stenosis in the posterior circulation or basilar artery at 6 months poststroke. Recurrent Intra-Luminal Aortic Thrombus And Ischemic Stroke In A Patient With Essential Thrombocythemia (P6.262)
CONCLUSIONS: This case highlights the importance of recognizing essential thrombocythemia as an entity that can lead to devastating complications, including acute coronary syndrome and either arterial and venous thrombosis. Early recognition and effective cytoreductive therapy are cornerstones to the prevention of recurrent embolism.
