Patent foramen ovale (PFO) closure to prevent recurrent stroke in a patient with a nickel allergy.
This article reviews current management of PFO in association with cryptogenic stroke in light of the results of three recent randomized controlled trials and two meta-analyses of observational data. The article will also discuss circumstances which merit consideration for closure despite the negative trial data, and will review our management strategy for closure in this patient with a nickel allergy that precluded the use of the most commonly used PFO closure device. Acute stroke from paradoxical embolism of dense fibrous tissue following pacemaker lead extraction: salvation by mechanical thrombectomy
Key Clinical Message Systemic embolization is a dreaded complication of transvenous lead extraction (TLE), even without visible vegetations. Preoperative patent foramen ovale evaluation is important, justifying neurological surveillance or consideration of surgical extraction in selected cases. In case of stroke after TLE, mechanical thrombectomy is a successful therapy, and should be readily available. Systemic embolization is a dreaded complication of transvenous lead extraction (TLE), even without visible vegetations. Sudden onset hemiplegia at high altitude
This report describes a case of posterior circulation infarct thought to be due to a paradoxical embolus through a patent foramen ovale. The relationship between high-altitude physiology, increased incidence of thromboembolism and the significance of patent foramen are discussed in the report.

To Reduce Stroke with PFO Closure, Respect the Shunt
We read with interest the very timely updated meta-analysis regarding closure of patent foramen ovale for cryptogenic stroke by Vaduganathan et  al.1 They suggest a marked benefit in stroke prevention with an increase in the incidence of atrial fibrillation. However, we would like to draw your attention to a few caveats that are important for clinicians dealing with this situation and expand on the pooled analysis using the same 5 trials i ncluded in the current paper.2-6

Platypnoea orthodeoxia syndrome and its association with cryptogenic ischaemic stroke
J R Coll Physicians Edinb. 2023 Mar 4:14782715231159470. doi: 10.1177/14782715231159470. Online ahead of print.ABSTRACTPlatypnoea orthodeoxia syndrome is a rare condition, which can be present in patients with patent foramen ovale (PFO). In this case, a 72-year-old lady presented to the emergency department with a cryptogenic stroke associated with right thalamic infarct. While in hospital, it was noticed that the patient had desaturations in the upright position, and these improved in the recumbent position, consistent with platypnoea orthodeoxia syndrome. The patient was found to have a PFO, which was then closed, and he...

' Hole in Heart ' Defect May Raise Stroke Risk After Surgery
TUESDAY, Feb. 6, 2018 -- People born with a hole in their heart face an increased risk for stroke after surgery, a new study finds. This common type of birth defect -- known as patent foramen ovale (PFO) -- is a hole between the upper chambers of...

Acute Ischemic Stroke in a Young Woman With No Known Risk Factors.
CONCLUSION: Onset of neurologic symptoms can be immediate or delayed in patients undergoing venous foam sclerotherapy. Early recognition of neurologic deficits resulting from paradoxical gas embolism and its treatment with hyperbaric oxygen can prevent permanent disability. PMID: 29677415 [PubMed - in process]
