Risk factors and pathogenic microorganism characteristics for pneumonia in convalescent patients with stroke: A retrospective study of 380 patients from a rehabilitation hospital
Stroke significantly impacts public health and ranks among the leading causes of death and disabilities, resulting in enormous costs measured in both health care resources and lost productivity. It results primarily from embolus or thrombosis for ischemic stroke and hypertension for hemorrhagic stroke, respectively [1]. In 2016, there were 5.5 million deaths and 116.4 million disability-adjusted life-years (DALYs) owed to stroke [2]. As reported, the impact of stroke on the Chinese population is more severe compared to average global levels, and the prevalence of stroke continues to surpass that of ischemic heart disease [3 –4].

The effects of combined early oral health care and early mobilisation on the incidence of stroke-associated pneumonia
Pneumonia after stroke, which influences mortality1 and duration of hospitalisation,2 has been a common critical issue in stroke patients. In 2015, the Pneumonia in Stroke Consensus Group proposed the diagnostic criteria of stroke-associated pneumonia (SAP), which is defined as a spectrum of lower respiratory tract infections in the first 7 days after stroke.3 A systematic review and meta-analysis reported that the incidence of SAP was 14%.4 There are several mechanisms of SAP onset, such as aspiration related to consciousness disturbance and dysphagia, stroke-induced immunodepression due to pathogenic bacteria, and immunological changes.

The Frequency of Poststroke Infections and Their Impact on Early Stroke Outcome
Introduction: Poststroke infections are the most common medical complications of stroke and can occur in up to 65% of patients. The aim of this study was to assess the rate of infectious complications during hospitalization of stroke patients and to evaluate the impact of infection in general, including each of the urinary tract infection (UTI), pneumonia, and sepsis, on fatal and poor functional outcome at discharge. Methods: This retrospective study enrolled patients who have been diagnosed with acute ischemic stroke treated in a 1-year period. Poor functional outcome at discharge was defined as severe invalidity and incl...

Outcomes of Thrombolytic Treatment for Acute Ischemic Stroke in Dialysis-Dependent Patients in the United States
Conclusions: The 2-fold higher odds of in-hospital mortality associated with administration of IV thrombolytics in dialysis-dependent patients who present with acute ischemic stroke warrant a careful assessment of risk–benefit ratio in this population.

External Validation of the Prestroke Independence, Sex, Age, National Institutes of Health Stroke Scale (ISAN) Score for Predicting Stroke-Associated Pneumonia in the Athens Stroke Registry
The Prestroke Independence, Sex, Age, National Institutes of Health Stroke Scale (ISAN) score was developed recently for predicting stroke-associated pneumonia (SAP), one of the most common complications after stroke. The aim of the present study was to externally validate the ISAN score.

Reduction in 30 Day Readmission of Stroke Patients from Acute Care Centers with the Involvement of Medical Hospitalists (P1.214)
Conclusions: In the absence of a medical hospitalist, stroke patients are predominantly readmitted for infectious etiology. Readmission complications are decreased among those discharged from a primary hospitalist service. We conclude that involving medical hospitalists in the care of stroke patients during their initial admission optimizes discharge condition; thus reducing the risk of complications and decreasing frequency of re-hospitalizations.Disclosure: Dr. Hua has nothing to disclose. Effect of Dysphagia Screening Strategies on Clinical Outcomes After Stroke: A Systematic Review for the 2018 Guidelines for the Early Management of Patients With Acute Ischemic Stroke AHA/ASA Systematic Review
Conclusions—There were insufficient RCT data to determine the effect of dysphagia screening protocols on reducing the rates of pneumonia, death, or dependency after stroke. Additional trials are needed to compare the validity, feasibility, and clinical effectiveness of different screening methods for dysphagia.

The Dysphagia in Stroke Protocol Reduces Aspiration Pneumonia in Patients with Dysphagia Following Acute Stroke: a Clinical Audit
AbstractCough reflex testing has been evaluated as a component of the clinical swallowing assessment as a means of identifying patients at risk of aspiration during swallowing. A previous study by our research group found good sensitivity and specificity of the cough reflex test for identifying patients at risk of aspiration post-stroke, yet its use did not decrease pneumonia rates, contrary to previous reports. The aim of this study was to expand on our earlier work by implementing a clinical management protocol incorporating cough reflex testing within the same healthcare setting and compare patient outcomes to those fro...

Predictors of Recurrent Ischemic Stroke in Obese Patients With Type 2 Diabetes Mellitus: A Population-based Study
Each year, approximately 0.8 million people are estimated to experience a new or recurrent stroke, according to the 2019 report of the American Heart Association.1 In Malaysia, stroke is the third leading cause of death after ischemic heart disease (IHD) and pneumonia.2 Both obesity and diabetes are considered as the established independent risk factors for ischemic stroke.3 Type 2 diabetes mellitus (T2DM) was also found to be associated with an increase in all-cause as well as cardiovascular mortality following the stroke,4,5 and in a meta-analysis of 18 studies, patients with diabetes had a higher stroke recurrence as co...

Predictors of poor outcome despite successful endovascular treatment for ischemic stroke: results from the MR CLEAN Registry
Conclusion Baseline patient factors and postprocedural adverse events are important predictors of poor functional outcome in successfully reperfused patients with ischemic stroke. This implies that prevention of postprocedural adverse events has the greatest potential to further improve outcomes in these patients.
