Rational clinical approach to transient ischaemic attack, stroke and associated mimics: A retrospective cohort study
CONCLUSIONS: The present study demonstrated that specific clinical features and risk factors were associated with the final diagnosis at TIA clinic. These clinical features may assist with diagnosis of TIA in centres without access to a vascular neurologist.PMID:37190670 | DOI:10.1111/1742-6723.14238

Mitral Valve Blood Cyst and Multi-Territory Ischaemic Stroke
A 78-year-old male with a history of hypertension and hypercholesterolaemia was admitted to hospital with acute left eye visual loss and diagnosed with retinal artery occlusion. Magnetic resonance imaging (MRI) revealed multiple, bilateral cerebral infarcts involving supra- and infra-tentorial brain consistent with acute ischaemic stroke from a central embolic source. Extended ambulatory electrocardiographic monitoring displayed sinus rhythm with no evidence of atrial flutter or fibrillation. Transthoracic echocardiography (TTE) revealed a cystic structure associated with the mitral valve, with no restriction or regurgitation.

Risk factors for ischemic cerebral stroke in patient with acute amaurosis fugax
CONCLUSION: Ischemic stroke and myocardial infarction were diagnosed in 0.3%-0.9% of hospitalized acute AF cases. Presence of hypercoagulable state, systemic vasculitis, CAD, and atherosclerosis each individually increased the risk of ischemic stroke by more than 3-fold; patients with these risk factors and acute AF should be closely monitored for developing acute systemic thrombotic events.PMID:36368408 | DOI:10.1016/j.jcjo.2022.10.010

Similarities and differences in systemic risk factors for retinal artery occlusion and retinal vein occlusion: A nationwide case-control study
CONCLUSION: Differences in the level of associations with the included risk factors suggests differences in the pathophysiologies of the two diseases. The main pathophysiology associated with RAO was atherosclerosis, whereas the main pathophysiology associated with RVO was changes in the pressure gradients of the eyes.PMID:36056288 | DOI:10.1007/s10792-022-02483-3

State of target organs damage and prevalence of clinical associated conditions in patients with a hypertensive crisis and uncontrolled hypertension. (pilot study)
CONCLUSIONS: Patients with hypertensive crisis have more pronounced damage of target organs, as well as the most frequent prevalence of associated clinical conditions.PMID:36027392 | DOI:10.1097/01.hjh.0000838492.53623.dc

Central Retinal Artery Occlusion Is Related to Vascular Endothelial Injury and Left Ventricular Diastolic Dysfunction
J Clin Med. 2022 Apr 18;11(8):2263. doi: 10.3390/jcm11082263.ABSTRACTCentral retinal artery occlusion (CRAO) is an emergency state characterized by sudden, painless vision impairment. Patients with CRAO have an increased risk of cardiovascular events, including stroke, likely related to vascular endothelial damage. Therefore, we investigated flow-mediated dilatation (FMD) of the brachial artery as a marker of endothelial dysfunction, intima-media complex thickness (IMT) of the common carotid artery, pointing to the arterial wall atherosclerotic alteration, and transthoracic echocardiographic parameters in 126 consecutive C...

Retinal parameters, cortical cerebral microinfarcts and their interaction with cognitive impairment
Conclusions: Retinal vascular and neuronal parameters are associated with cortical CMIs and persons with both pathologies are likely to have cognitive impairment. Further studies may be warranted to evaluate the clinical utility of retinal parameters and CMI in risk prediction for cognitive dysfunction.Data access statement: The data that support the findings of this study is available from the corresponding author upon request.PMID:35450485 | DOI:10.1177/17474930221097737

Clinical risk factors for retinal artery occlusions: a nationwide case-control study
CONCLUSION: The investigated risk factors suggest that atherosclerosis and conditions changing the intraocular pressure are involved in the pathophysiology of RAO.PMID:35305540 | DOI:10.1007/s10792-022-02247-z

Retinal microvascular abnormalities and risks of incident stroke and its subtypes: The Circulatory Risk in Communities Study
Conclusion: Retinal microvascular abnormalities were positively associated with the risk of incident stroke in the general Japanese population. Routine retinal photography could provide positive clinical insights into stroke risk stratification independent of blood pressure, antihypertensive medication use, and other risk factors.

Current Guidelines on Management of Amaurosis Fugax and Transient Ischemic Attacks
Asia Pac J Ophthalmol (Phila). 2022 Feb 24. doi: 10.1097/APO.0000000000000511. Online ahead of print.ABSTRACTTransient ischemic attack (TIA) is defined as a transient episode of neurological dysfunction resulting from focal brain, spinal cord, or retinal ischemia, without associated infarction. Consequently, a TIA encompasses amaurosis fugax (AF) that is a term used to denote momentary visual loss from transient retinal ischemia. In this review, we use the word TIA to refer to both cerebral TIAs (occurring in the brain) and AF (occurring in the retina). We summarize the key components of a comprehensive evaluation and mana...

Incorporation of Retinal Arteriolosclerosis into Risk Stratification of Blood Pressure Category According to the 2017 ACC/AHA Blood Pressure Guideline
CONCLUSION: Incorporation of the assessment for RA may facilitate the CVD risk stratification of people classified based on the 2017 ACC/AHA BP guideline, particularly for those categorized in stage 1 hypertension.PMID:34866070 | DOI:10.5551/jat.63262
