Influence of cerebral white matter lesions on the activities of daily living of older patients with mild stroke

Outcome of intravenous recombinant tissue plasminogen activator for acute ischemic stroke in patients aged over 80 years
ConclusionsRt‐PA therapy would appear to improve prognosis in the elderly with ischemic stroke. This suggests that age alone should no longer be a barrier to rt‐PA therapy. Geriatr Gerontol Int 2015; ●●: ●●–●●.

Loss-of-function mutation in ABCA1 and risk of Alzheimer's disease and cerebrovascular disease
Conclusions A loss-of-function mutation in ABCA1, present in 1:500 individuals, was associated with low plasma levels of apoE and with high risk of AD and cerebrovascular disease in the general population.

Incidence of stroke and seizure in Alzheimer's disease dementia
Background: the objective of the study was to estimate and compare the incidence rates of ischaemic and haemorrhagic stroke and seizure among cohorts with and without Alzheimer's disease (AD) dementia. Methods: we conducted a retrospective cohort study using electronic medical records (EMRs) from primary care practices that participated in The Health Improvement Network (THIN) in the United Kingdom from 1 January 1990 to 31 July 2009. For each AD-dementia patient, we selected one general population control patient without AD-dementia matched to one AD-dementia patient on year of birth, sex and physician practice. Findings:...

Older age is a strong predictor for poor outcome in intracerebral haemorrhage: the INTERACT2 study
Conclusion: In the INTERACT2 cohort, older people had more severe ICH and worse outcomes (death, major disability and HRQoL). These data will help guide clinicians manage older people with haemorrhagic stroke. Clinical Trial Registration: http://www.clinicaltrials.gov (NCT00716079).

Intravenous Thrombolysis for Acute Ischemic Stroke in the Elderly: An Italian Cohort Study in a “Real World” Setting
Conclusion Thrombolysis in patients aged >80 years was not associated with significantly increased intracranial hemorrhage. The higher 3-month disability rate observed in octogenarians may be explained by the more severe stroke and higher poststroke disability. Based on the current, “real world setting” study, we advocate the need for a randomized clinical trial to better clarify the efficacy and safety of intravenous thrombolysis for acute ischemic stroke in the elderly.

Total and Differential White Blood Cell Counts in Late Life Predict 8‐Year Incident Stroke: The Honolulu Heart Program
ConclusionIn elderly Japanese‐American men, higher total WBC and neutrophil counts were independent predictors of overall stroke, as well as thromboembolic stroke.

PGE2 receptor agonist misoprostol protects brain against intracerebral hemorrhage in mice.
In this study, we tested the efficacy of misoprostol in the 12-month-old mice subjected to 1 of 2 complementary ICH models, the collagenase model (primary study) and blood model (secondary study, performed in an independent laboratory). We also investigated its potential mechanism of action. Misoprostol posttreatment decreased brain lesion volume, edema, and brain atrophy and improved long-term functional outcomes. In the collagenase-induced ICH model, misoprostol decreased cellular inflammatory response; attenuated oxidative brain damage and gelatinolytic activity; and decreased high-mobility group box 1 (HMGB1) expressio...

Vitamin K Antagonists and Cognitive Impairment: Results From a Cross-Sectional Pilot Study Among Geriatric Patients
Conclusions. We found more frequent cognitive impairment associated with the use of VKAs, specifically fluindione, among geriatric patients.

Safety of intravenous thrombolysis for ischaemic stroke in Asian octogenarians and nonagenarians
Conclusion: octogenarians and nonagenarians receiving IVT have a similar safety outcome. Asian octogenarians and nonagenarians receiving IVT for ischaemic stroke are not at higher risk of symptomatic ICH and mortality than Western patients.

Delirium Risk Prediction, Healthcare Use and Mortality of Elderly Adults in the Emergency Department
ConclusionThis risk prediction rule may help identify a group of individuals in the ED at high risk of developing delirium who should undergo screening, but it requires external validation. Identification of delirium in the ED may enable physicians to implement strategies to decrease delirium duration and avoid inappropriate discharge of individuals with acute delirium, improving outcomes.

Thrombolytic therapy with intravenous recombinant tissue plasminogen activator in Japanese older patients with acute ischemic stroke: Fukuoka Stroke Registry
ConclusionsIn patients treated with rt‐PA, old age is associated with poor functional outcome and in‐hospital mortality. However, the probability of neurological improvement and symptomatic intracranial hemorrhage in older patients is comparable with that in younger patients. Geriatr Gerontol Int 2013; ●●: ●●–●●.

Intravenous thrombolysis in patients aged more than 80 years in the three rural hospitals in southeast Poland: An observational study
ConclusionsOlder age affected the increased times for pre‐ and in‐hospital procedures in rural patients with AIS. Older patients should not be denied the chance to benefit from IV‐thrombolysis, but because of the higher mortality, the application of this therapy should be carefully decided on.
