Is the outpatient management of acute minor stroke feasible and safe?
Paul et al1 have assessed the clinical outcomes, early hospital admission rates and hospital care costs in clinic referred and hospital referred minor stroke patients in a prospective population based study. They did not find significant differences in the 30 day admission rate in clinic patients compared with the 30 day readmission rate after discharge in hospital treated patients (16/237 vs 9/150). The 30 day recurrent stroke risk in patients with minor ischaemic stroke was also similar in those discharged from clinic compared with hospital patients (3.8% vs 5.3%; p=0.61). ConclusionsMMD is presented most commonly with ischemic stroke in the pediatric age group and with hemorrhagic stroke in the adult age group. MMD-associated intracranial aneurysms frequently occur in patients presenting with hemorrhagic unilateral MMD and are associated with an extremely high rate of rupture. In 2015, multiple randomized controlled trials clearly demonstrated that mechanical thrombectomy could save lives and prevent disability in patients with large vessel acute ischemic stroke. Vascular closure devices in stroke patients receiving tissue plasminogen activator: A retrospective analysis from an academic tertiary medical center and a teaching community hospital stroke database
Stroke is the fourth leading cause of death in the United States [1]. Recanalization, or opening, of the blocked artery in ischemic stroke patients can limit the extent of infarction and significantly improve a patient's outcome [2]. In 1995, NINDS tPA (tissue plasminogen activator) trial suggested that tPA administered intravenously within three hours of stroke onset could improve functional outcomes in patients [3]. Since then, acute stroke therapy has been in constant evolution with the universal goals of improving outcomes and reduction in complications.

Outcomes of Endovascular Thrombectomy Performed 6-24 h after Acute Stroke from Extracranial Internal Carotid Artery Occlusion.
This study investigated the outcomes of thrombectomy performed 6-24 h after stroke onset caused by extracranial ICA occlusion. Of 586 stroke patients receiving thrombectomy during the past 3 years and registered in the Tama Registry of Acute Endovascular Thrombectomy database, 24 were identified with ICA occlusion (14 extracranial and 10 intracranial), known to be well 6-24 h before presentation, and with pre-stroke modified Rankin Scale (mRS) score of 0 or 1.
