Monitoring risk and preventing ischemic stroke in the very old
This article examines current guidelines and evidence regarding medical management, lifestyle changes, and psychosocial interactions that can contribute to the primary and secondary prevention of ischemic stroke in the very old.AREAS COVERED: The authors conducted a literature search for ischemic stroke prevention and risk assessment in the elderly via PubMed. Furthermore, they describe current strategies for monitoring risk and preventing ischemic stroke in the elderly population.EXPERT OPINION: Ischemic stroke poses a significant health risk to the elderly, with prevention relying on managing modifiable risk factors such...

E-065 Interim safety and occlusion outcomes of endovascular treatment of large intracranial aneurysms in the sterling registry
ConclusionEarly results from the STERLING registry for large aneurysms are promising. Adequate occlusion per Independent Core Lab was achieved in 86.7% and retreatment rates were relatively low over 1-year follow-up. A good functional outcome was achieved in 85.7% of patientsDisclosures O. Investigation of Combined Carotid Endarterectomy and Coronary Artery Bypass Graft Surgery Outcomes and Adverse Event Risk Factors in the Vascular Quality Initiative
CONCLUSIONS: Combined CEA and CABG provides excellent long-term mortality prevention in patients with co-existing severe coronary and carotid atherosclerosis. Simultaneous CEA and CABG provides equivalent stroke prevention and long-term survival to both a cohort of patients undergoing coronary revascularization within 5 years of CEA and patients undergoing isolated CEA or CABG in the literature. The two most impactful modifiable risk factors towards long-term stroke and mortality prevention for patients undergoing simultaneous CEA-CABG are patch placement at CEA site and adherence to statin medication therapy.PMID:37303074...

Evidence That Increasing Serum 25(OH)D Concentrations to 30 ng/mL in the Kingdom of Saudi Arabia and the United Arab Emirates Could Greatly Improve Health Outcomes
Biomedicines. 2023 Mar 23;11(4):994. doi: 10.3390/biomedicines11040994.ABSTRACTAccumulating evidence supports the potential protective effects of vitamin D against chronic diseases such as Alzheimer's disease, autoimmune diseases, cancers, cardiovascular disease (ischaemic heart disease and stroke), type 2 diabetes, hypertension, chronic kidney disease, stroke, and infectious diseases such as acute respiratory tract diseases, COVID-19, influenza, and pneumonia, as well as adverse pregnancy outcomes. The respective evidence is based on ecological and observational studies, randomized controlled trials, mechanistic studies, ...

Aetiology of extracranial carotid free-floating thrombus in a prospective multicentre cohort
CONCLUSIONS: The majority of symptomatic carotid artery FFT are likely caused by local plaque rupture, more than half of which are associated with moderate to severe carotid stenosis requiring revascularisation. However, a significant number of FFTs are caused by non-atherosclerotic mechanisms warranting additional investigations.PMID:36368714 | DOI:10.1136/svn-2022-001639

Brain bioenergetics in chronic hypertension: Risk factor for acute ischemic stroke
Biochem Pharmacol. 2022 Sep 28;205:115260. doi: 10.1016/j.bcp.2022.115260. Online ahead of print.ABSTRACTChronic hypertension is one of the key modifiable risk factors for acute ischemic stroke, also contributing to determine greater neurological deficits and worse functional outcome when an acute cerebrovascular event would occur. A tight relationship exists between cerebrovascular autoregulation, neuronal activity and brain bioenergetics. In chronic hypertension, progressive adaptations of these processes occur as an attempt to cope with the demanding necessity of brain functions, creating a new steady-state homeostatic ...
