Contemporary Review on Craniectomy and Cranioplasty; Part 1: Decompressive Craniectomy
This paper aims to review clinical benefits of decompressive craniectomy (DC) in both adult and paediatric populations; its indications and factors contributing to its postoperative success. The Glasgow Outcome Scale and the Modified Rankin Scale are the most commonly used scales to assess the long-term outcome in patients post DC. In adult traumatic brain injury patients, 2 randomized clinical trials were carried out; DECRA (Decompressive Craniectomy in Diffuse Traumatic Brain Injury) and RESCUEicp (Randomised Evaluation of Surgery with Craniectomy for Uncontrollable Elevation of inter cranial pressure) employing collecti...

Visit-to-visit blood pressure variability and the risk of stroke in the Netherlands: A population-based cohort study
by Alis Heshmatollah, Yuan Ma, Lana Fani, Peter J. Koudstaal, M. Arfan Ikram, M. Kamran Ikram BackgroundApart from blood pressure level itself, variation in blood pressure has been implicated in the development of stroke in subgroups at high cardiovascular risk. We determined the association between visit-to-visit blood pressure variability and stroke risk in the general population, taking into account the size and direction of variation and several time intervals prior to stroke diagnosis. Methods and findingsFrom 1990 to 2016, we included 9,958 stroke-free participants of the population-based Rotterdam Study in the Nethe...

The WOEST  2 registry : A prospective registry on antithrombotic therapy in atrial fibrillation patients undergoing percutaneous coronary intervention
CONCLUSIONS: Patients on combined oral anticoagulation and antiplatelet therapy undergoing PCI are elderly and have both a high bleeding and ischaemic risk. Over time, a NOAC plus clopidogrel became the preferred treatment. The rate of thrombotic and bleeding events was not significantly different between patients on triple or dual therapy or between those on VKAs versus NOACs.PMID:35230636 | DOI:10.1007/s12471-022-01664-0

Endovascular Thrombectomy in Young Patients With Stroke: A MR CLEAN Registry Study
CONCLUSIONS: Ten percent of patients with acute ischemic stroke undergoing endovascular thrombectomy were younger than 50. Cardioembolism and carotid dissection were common underlying causes in young patients. In one-third of cases, no cause was identified, indicating the need for more research on stroke cause in young patients. Young patients had better prognosis and lower risk of symptomatic intracranial hemorrhage than older patients.PMID:34872339 | DOI:10.1161/STROKEAHA.120.034033

Quantified health and cost effects of faster endovascular treatment for large vessel ischemic stroke patients in the Netherlands
Conclusion One-minute faster EVT increases QALYs while cumulative costs remain largely unaffected. Therefore, faster EVT provides better value of care at no extra healthcare costs.

Access to stroke care in Europe: results of a vignette approach
ConclusionsWhile stroke related services are formally covered in almost all countries, considerable access problems exist in several countries. Future cross-country surveys of access to stroke care should collect quantitative data related to proportions of patients receiving particular services as well as qualitative data on access barriers.

Antithrombotic therapy in high-risk patients  after percutaneous coronary intervention; study design, cohort profile and incidence of adverse events
CONCLUSION: The high risk for both bleeding and ischaemic events in this cohort of patients with multiple clinical risk factors illustrates the challenges that the cardiologist faces to make a balanced decision on the optimal treatment strategy. This cohort will serve to answer several future research questions about the optimal management of these patients on dual/triple antithrombotic therapy, and the possible value of a wide range of laboratory tests to guide these decisions.PMID:34468944 | DOI:10.1007/s12471-021-01606-2

Screening and follow-up care for cognitive and emotional problems after transient ischaemic attack and ischaemic stroke: a national, cross-sectional, online survey among neurologists in the Netherlands
Conclusions This study demonstrates that clinical practice at neurology departments is highly variable regarding screening, information provision and follow-up care for cognitive and emotional problems in patients after TIA or ischaemic stroke. Approximately half of the participating neurologists reported that screening was performed only sometimes or never for cognitive and emotional problems after TIA and ischaemic stroke.

E-081 Susceptibility - Weighted imaging findings in experimental acute ischemic stroke model
ConclusionIn the hyperacute stages of MCAO, SWI images display a consistent and significant signal drop relative to the unaffected side. This signal drop is suspected to result from the paramagnetic effect of deoxyhemoglobin and venodilation and thus an indirect means of visualizing tissue hypoxia.Disclosures N. Saadat: None. G. Influence of Onset to Imaging Time on Radiological Thrombus Characteristics in Acute Ischemic Stroke
Conclusion: In our population of acute ischemic stroke patients, we found no clear association between onset-to-imaging time and radiological thrombus characteristics. This suggests that elapsed time from stroke onset plays a limited role in the interpretation of radiological thrombus characteristics and their effect on treatment results, at least in the early time window.

Call Characteristics of Patients Suspected of Transient Ischemic Attack (TIA) or Stroke During Out-of-Hours Service: A Comparison Between Men and Women
Conclusion: Overall, patient and call characteristics are mostly comparable between men and women, and these only modestly assist in identifying TIA/stroke. There were no gender differences in allocated urgencies after telephone triage in patients with TIA/stroke.
