911 Calls for Emergency Medical Services in Heart Failure: A Descriptive Qualitative Study
CONCLUSIONS: Overall, symptoms and signs of HF exacerbation were common reasons to activate 911 calls. Falls were frequently reported. Under the duress of the emergent situations surrounding the 911 call, callers rarely mentioned the existence of HF. Interventions are needed to guide patients with HF and their family members to promote the management of HF to reduce EMS activation as well as to activate EMS quickly for acute changes in HF conditions.PMID:34935743 | DOI:10.1097/JCN.0000000000000861

Factors Associated With Cognitive Impairment in Heart Failure With Preserved Ejection Fraction
Background Cognitive impairment is prevalent in heart failure and is associated with higher mortality rates. The mechanism behind cognitive impairment in heart failure with preserved ejection fraction (HFpEF) has not been established. Objective The aim of this study was to evaluate associations between abnormal cardiac hemodynamics and cognitive impairment in individuals with HFpEF. Methods A secondary analysis of Atherosclerosis Risk in Communities (Atherosclerosis Risk in Communities) study data was performed. Participants free of stroke or dementia who completed in-person assessments at visit 5 were in...

Multimorbidity in Patients With Acute Coronary Syndrome Is Associated With Greater Mortality, Higher Readmission Rates, and Increased Length of Stay: A Systematic Review
Conclusions Multimorbid patients with ACS are at a greater risk for worse outcomes than their nonmultimorbid counterparts. Lack of consistent measurement makes interpretation of the impact of multimorbidity challenging and emphasizes the need for more research on multimorbidity's effects on postdischarge healthcare utilization.

An Integrative Review of Brain-Derived Neurotrophic Factor and Serious Cardiovascular Conditions
Discussion These findings indicate an emerging area of science. Future investigation is needed on serious cardiovascular condition phenotypes in relationship to BDNF in the same study conditions. Results also suggest for use of standardized BDNF measurement across studies and additional investigation in cardiovascular inflammatory processes that affect BDNF. Moreover, within specific populations, the frequency of Met alleles may be too low to be detected in sample sizes normally found in these types of studies.

Factors Associated With Cognitive Impairment in Heart Failure With Preserved Ejection Fraction.
CONCLUSIONS: Cognitive impairment is prevalent in HFpEF and affects several cognitive domains. The current study supports the importance of cognitive screening in patients with heart failure. An association between abnormal cardiac hemodynamics and cognitive impairment was observed, but other factors are likely involved. PMID: 32649377 [PubMed - as supplied by publisher]

The Symptom Experience of Patients With Atrial Fibrillation Before Their Initial Diagnosis
Conclusions The current study identified challenges participants experienced in developing awareness of their AF symptoms, the complex cognitive processes associated with evaluation, and barriers that made it difficult to respond to AF symptoms in a timely manner. Understanding the prediagnosis symptom experience from the patient's perspective is essential for the enhancement of current AF screening practices.

Home Health Agency Factors Associated With Acute Care Hospitalization and Emergency Department Use
We linked the Medicare Provider Utilization and Payment Data for Home Health and the Home Health Compare data for the year 2016 to identify home healthcare agency (HHA) characteristics associated with acute care hospitalization (ACH) or emergency department (ED) use. The study cohort consisted of 9,800 HHAs. Beta regression was used to examine the association between average age, race/ethnic composition, number of skilled nursing visits, number of therapy visits, percentage of dual eligible patients, HHA ownership, HHA location, Medicare tenure, proportion of patients with a diagnosis of schizophrenia, stroke, diabetes, de...

Causes of hospital admissions and deaths among Brazilian elders between 2005 and 2015.
CONCLUSION: Circulatory and respiratory diseases were the main causes of hospital admissions and death among the elderly, highlighting the impotant increase in pneumonia as a cause of morbimortality. PMID: 31644715 [PubMed - in process]

Common Postcardiothoracic Surgery Arrhythmias
Cardiac arrhythmias are common after cardiac surgery and have profound sequelae. Bradycardias are typically transient and have reversible causes; however, persistent atrioventricular block is an indicator for permanent pacemaker implantation after valvular surgery. Transcatheter aortic valve surgery is associated with even higher rates of permanent pacemaker implantation. Atrial fibrillation, the most common postoperative arrhythmia, is associated with ischemic stroke, myocardial infarction, congestive heart failure, and short-term mortality. Ventricular arrhythmias have extremely high in-hospital mortality, as well as lon...

Preventing Broken Hearts
Atherosclerotic cardiovascular disease (ASCVD) remains the most frequent cause of death in women throughout the world. More than 250,000 women die annually in the United States from ASCVD; mortality and morbidity have significantly increased since 1980.1,2 Women are more likely to develop heart failure or die within 5 years of a myocardial infarction (MI) and have a higher risk of stroke than men.2 Although it has been known for some time that women are at significant risk for developing ASCVD, women remain untreated or undertreated.

Effects of introducing a fee for “bed blockers” on adverse events among patients hospitalized for chronic obstructive pulmonary disease, heart failure, hip fracture and stroke
Conclusion: Overall, there is no strong evidence to suggest that the Norwegian healthcare Coordination Reform is functioning in a manner that exacerbates risk for readmissions or death among the patients considered in this study. Even so, patients with heart failure appear somewhat vulnerable.   Published on 2018-10-23 00:00:00
