EXPRESS: Magnetic resonance imaging for diagnostic workup of Embolic Stroke of Undetermined Source: a systematic review
CONCLUSION: MRI techniques allow to identify additional cardiac and vascular embolic sources and may further decrease the prevalence of patients with the diagnosis of ESUS.PMID:37435743 | DOI:10.1177/17474930231189946

Atrial cardiomyopathy: from cell to bedside
AbstractAtrial cardiomyopathy refers to structural and electrical remodelling of the atria, which can lead to impaired mechanical function. While historical studies have implicated atrial fibrillation as the leading cause of cardioembolic stroke, atrial cardiomyopathy may be an important, underestimated contributor. To date, the relationship between atrial cardiomyopathy, atrial fibrillation, and cardioembolic stroke remains obscure. This review summarizes the pathogenesis of atrial cardiomyopathy, with a special focus on neurohormonal and inflammatory mechanisms, as well as the role of adipose tissue, especially epicardia...

Modification of ventriculo –arterial coupling by spironolactone in nonischemic dilated cardiomyopathy
ConclusionsIn NIDCM, adding MRA to conventional anti ‐failure therapy markedly improved LV ejection fraction and reduced peripheral vascular resistance, due to both improved LV contractility and especially to enhanced V–A coupling, as Ea decreased to normal. Total circulatory performance was a sensitive indicator of both LV pump performance and th e arterial loading conditions.

Management and outcomes of hypertrophic cardiomyopathy in young adults
CONCLUSIONS: In young adults with HCM, MACE are common in the short term, especially in obstructive HCM and women, mostly arrhythmic in origin. Prophylactic ICD implantation is frequent and does not strictly follow the guidelines, while the use of European/USA guidelines is helpful but imperfect in identifying SCD risk.PMID:33744178 | DOI:10.1016/j.acvd.2020.12.006
