Spinal Cord and Cranial Bing-Neel Syndrome Complicated by Stroke (P6.018)
CONCLUSIONS: Our case highlights a novel presentation of Bing-Neel Syndrome that responded well to systemic and intrathecal therapies. Bing-Neel Syndrome should be considered in patients with Waldenström’s macroglobulinemia who present with central nervous system ischemia and/or myelopathy as early recognition and treatment may prevent development of irreversible neurological damage. Recurrent ischemic stroke related to Waldenström's macroglobulinemia
Waldenström's macroglobulinemia is a low-grade lymphoplasmacytic lymphoma which was first described in 1944. The disease is characterized by neoplastic proliferation of B lymphocytes and excessive production of monoclonal immunoglobulin M (IgM). The leading clinical features are lymphadenopathy and/or splenomegaly, anemia and hyperviscosity syndrome.

Spinal cord and cranial Bing-Neel Syndrome complicated by cerebral ischemia: A case report
Waldenström's macroglobulinemia (WM) is a B-cell malignancy characterized by lymphoplasmacytic lymphoma (LPL) within the bone marrow and an IgM monoclonal gammopathy in the blood. Symptoms may include weakness or fatigue, anorexia and weight loss, bleeding from the gums and nose, headache, blurred vision, peripheral neuropathy, and even stroke from hyperviscosity. The direct infiltration of the central nervous system (CNS), either as a diffuse leptomeningeal form or tumoral form is an exceedingly rare complication of WM, recognized as Bing-Neel Syndrome (BNS) [1].

Conclusions:Diagnostic approach to bilateral FNP should evaluate for: traumatic (skull fractures), infectious (classically Lyme disease), metabolic (diabetes), autoimmune (sarcoidosis, Guillain-Barré syndrome), congenital (Moebius syndrome) and neoplastic (brainstem tumors) entities.
