Radiation therapy for intracardiac metastasis

my patient 56 yo known to have a history of oligometastatic STS, recently presented with a secondary lesion in the heart. He was strictly asymptomatic.
we decided to go forward with systemic treatment not considering radiation in this setting.

When I graduated med school in 2010 it was a super competitive field and everyone made tons of money. I thought about it but didn't think I was competitive enough to get a spot. Here are the recent office notes from a dermatologist.

nodal relapse 9y after RP and PORT for prostatic carcinoma
75 years old patient, with controlled diabetes and hypertension, very fit.
he underwent in 2009, a radical prostatectomy followed 30 fractions of adjuvant 3D conformal radiation therapy to the prostatic bed only.
in 2018: PSA rise to 2.16
2019: PSA 2.16
Pet PSMA showed a 7 mm LN at the level of L4-5 secondary lesion, with no recurrence in the tumor bed, neither in bone or other LNs.
