SCS trial, splenectomy
78 yo M with FBSS and he's s/p splenectomy on acyclovir, fluconazol and PRN levofloxacin. He's got chronic leukopenia. Scheduled for trial next week and WBC 1.0 today. PLT normal. What does everyone do in this situation? He's miserable. Thinking move forward with the trial and be meticulous with sterile technique. Hibiclens BID x 3d prior. Keflex PO TID during the trial, which would be shortened to 3 days. Reasonable? (Source: Student Doctor Network)

Lack of education about fluoroquinolones?
I'm curious as to why this is taught so poorly, at least from my perspective. I didn't get much info in molecular pharmacology nor antimicrobial classes. even after asking the professors they shrugged it off. it seems like these antibiotics keep hitting the news, permanent nerve damage, disability from tendon rupture (not just achilles). I've trolled NCBI for specific studies and it's surprising how much information is out there on ciprofloxacin/levofloxacin etc that's not taught in any... Lack of education about fluoroquinolones? Hi! My name is Krystin, I'm 31 with two kids and here's my story. December 2012, I was at the beach enjoying myself in a hot tub. Two days later, I started coughing. Productive coughing. 2% Bactroban as nasal rinse
I saw a sinus specialist at Hopkins today who works with CF patients. I still have no diagnosis; the adult CF clinic sent me to her as they don't think I have CF. I was using pulmicort in my nose for the last month and today she switched to bactorban 2% mixed in a neil med solution, twice a day. I keep getting green drainage (specks, dried up muscus) My sinus scan looked pretty good after a month of levaquin and pulmicort. Anybody ever used this stuff in their nose?
