Recurrent Performance Problems
I'm a PGY-1, and with half of the year done and about 2 months of total actual EM time done, I'm noticing some pretty big recurrent problems. I was hoping to get some advice on how to deal. Firstly, I have a really bad habit of anchoring to diagnoses. Fairly often I'll get a strong bias towards whatever diagnosis seems most likely initially. The other day, a lady with history of kidney stones came in with colicky groin pain and couldn't get comfortable in any position. I got the workup... Urolithiasis
I'm confused here, both in step up 2 medicine and my school ( as far as I remember) said that KUB X-ray is the best initial test but in uworld it's said that either spiral CT or U/S is the imaging of choice, so who's right?!!!! When you suspect kidney stones
When do you get a CT scan? I've heard and seen everything from: always, first time they've had stone, systemically ill, hydronephrosis on bedside ultrasound. I've also seen people with mild hydro and suspected stone go home. The practice pattern varies wildly and I'm trying to gain a better understanding of what is important and what is not. "Pranic Healing" and a "Therapy Harp" as 1st line treatment for pain from fractures...
Almost fell over reading this front page article from NY Times. While some of the therapies such as massage/local nerve blocks have evidence to back them up, other treatments at this New Jersey ER seem down right nuts. IV Lidocaine for pain from kidney stones?!?! Clinical case: pre-op EKG or no?
67 yo male after hours in small hospital for kidney stones. Urologist wants to place stent. H/O CAD s/p stent about 8 yrs ago, HTN, CKD. Per pt and his daughter in pre op hold, pt works regularly at a store, denies CP/SOB with activities (pt relays that he had a penile implant and engages in regular sexual activity to you on the side because he doesn't want his daughter to hear).
