Myth and evidence in palatal expansion
Untruths and distortion of scientific findings inhabit the medical and dental fields, not intentionally but often because of the lack of firm evidence. The aim in this paper is to visit a common orthodontic treatment modality, palatal expansion, and explore whether the accumulated biologic and mechanical findings are sufficient or misinterpreted for proper intervention. For nearly a century, orthodontists have mainly focused on occlusion as the primary objective of orthodontic therapy. The introduction of the soft tissue paradigm has shifted treatment planning from hard tissue to soft tissue and reinforced the key role of facial esthetics in orthodontic diagnosis and treatment planning. Whereas occlusion is rooted in measurable definitions of Class I and other “keys to occlusion”, esthetics is more subjective, based not only on the orthodontist's assessment but also on personal and cultural preferences rather than rigid norms. The surge of finite element analysis in the study of orthodontic mechanics: are the findings applicable in practice?
Finite element (FE) analysis has emerged as the primary tool to investigate complex problems that are difficult to test clinically on living tissues, yielding valuable but not necessarily applicable information on orthodontic treatment modalities. Many studies are being published but the findings are difficult to interpret, lack in relevance, or cannot be critically appraised. In this paper, the FE modeling design and impact on the quality and outcome of research are discussed, with focus on how to narrow the gap between theory and clinical application. Treatment of Class II malocclusion in the growing patient: Early or late?
The aim of this paper is to evaluate several factors that can improve the efficacy of Class II treatment in the growing patient with mandibular skeletal retrusion. Previous research has shown that functional appliances are most effective in altering short-and long-term mandibular growth and mandibular sagittal position if active treatment includes the pubertal growth spurt.
