Pulsed ‐field ablation does not induce esophageal and periesophageal injury—A new esophageal safety paradigm in catheter ablation of atrial fibrillation
ConclusionIn contrast to thermic ablation tools, PFA is not associated with relevant esophageal and periesophageal injury, and might, therefore, reduce or eliminate the risk of potentially lethal AEF in interventional treatment of AF. The etiology of ablation-induced periesophageal edema is unknown but has not been shown to be related to lesion progression. (Source: Journal of Cardiovascular Electrophysiology)

Catheter ablation of typical atrial flutter improves cardiac chamber size and function
ConclusionsPatients who underwent CTI dependent AFL ablation showed an improvement in cardiac size and function at follow-up evaluation. While long-term results are still unknown, these findings indicate that restoration of sinus rhythm in patients with typical AFL is associated with improvement in atrial size and left ventricular function. Bipolar ablation of ventricular arrhythmias: Step ‐by‐step
AbstractRadiofrequency (RF) ablation of intramural ventricular arrhythmias (VAs) may require advanced ablation techniques to achieve effective energy transfer to the targeted tissue. As an alternative to standard RF ablation, catheter ablation can also be conducted in bipolar configuration when two ablation catheters participate in the RF circuit. This strategy has proved to result in deeper lesion formation and may be effective for eliminating arrhythmias that have been refractory to standard ablation. In this article, we provide a step-by-step guide on when and how to perform bipolar ablation of VAs. (Source: Journal of ...

Sensing of concealed ventricular extrasystoles by pacemakers. Fifty years later
AbstractWhether a pacemaker can sense concealed ventricular extrasystoles still remains debatable since its occurrence was first proposed in 1972. It must remain a diagnosis of exclusion if it really exists. Isoelectric complexes and all the causes of oversensing especially discrete false signals generated by a defective pacemaker lead must be excluded before concealed ventricular extrasystoles can be postulated. The appearance of new accessory pathway after the Cone procedure
ConclusionThe appearance of a new AP after the reconstruction of CHD is a rare finding and should raise suspicion of an acquired AP connection. (Source: Journal of Cardiovascular Electrophysiology)

Left atrial appendage occlusion for atrial fibrillation and bleeding diathesis
ConclusionsShort-term anti-thrombotic drugs may not be needed after LAAO implant in patients with high bleeding risk and could be harmful. Larger, prospective studies would be warranted to test these findings. (Source: Journal of Cardiovascular Electrophysiology)

Left bundle branch pacing versus left ventricular septal pacing as a primary procedural endpoint during left bundle branch area pacing: Evaluation of two different implant strategies
ConclusionThe LBBP strategy was associated with comparable implant parameters than the LVSP strategy but resulted in higher rates of septal perforation. Proven LBB capture and LVSP showed comparable effects on LVEF during follow-up. (Source: Journal of Cardiovascular Electrophysiology)

Right sided approach for left bundle branch pacing using lumen ‐less lead: Technical considerations and follow‐up outcome
ConclusionRight sided LBBP is feasible, safe and effective in patients requiring pacing for symptomatic bradyarrhythmia and CRT. Further development in dedicated tools for right-sided approach would help in reducing the fluoroscopy-duration and radiation-dose. (Source: Journal of Cardiovascular Electrophysiology)

Impact of pulsed field ablation on intraluminal esophageal temperature
ConclusionA small but significant intraluminal esophageal temperature rise can be observed in most patients during PFA. TESO rise over 40 °C is rare. The clinical implications of the observed findings need to be further evaluated. (Source: Journal of Cardiovascular Electrophysiology)

Impact of intracardiac echocardiography versus transesophageal echocardiography guidance on left atrial appendage occlusion procedures: A meta ‐analysis
ConclusionICE guidance is associated with similar procedural efficacy and safety, but could result in improved lab efficiency (as established by a significant reduction in in-room time). No differences in the rate of periprocedural leaks were found. A higher prevalence of residual interatrial septal defects was observed with ICE guidance.
