Considerations in Salvage Laryngectomy Reconstruction
Pharyngocutaneous fistula (PCF) is a dreaded complication from total laryngectomy (TL), as it can cause wound issues, lengthen hospitalization, increase costs, and prolong nothing-by-mouth status. In this issue of JAMA Otolaryngology –Head& Neck Surgery, Moreno et al evaluated 309 patients from multiple institutions who underwent salvage TL. This work nicely focuses on the subset of TL with limited pharyngeal defect amenable to primary closure (PC), distinguishing it from other work on this topic. The authors report a PCF rate of 29%. In their study, the investigators evaluated the use of free tissue transfer (FTT) v...

Neoadjuvant Chemotherapy and Transoral Robotic Surgery for Oropharynx Cancer and Quality of Life
To the Editor To deintensify treatment (avoid overtreatment) for patients with human papillomavirus virus –associated oropharyngeal squamous cell carcinoma (HPV-OPSCC) is certainly one of the hot topics in head and neck cancer, driven by the goal of enhancing quality of life. Although various strategies are under exploration, the unconventional inclusion of neoadjuvant chemotherapy by Silver et al has captured our attention.
