Nutritional Care for Patients with Intestinal Failure
Intestinal failure is a debilitating, complex disorder associated with loss of portions of intestine or loss of intestinal function. Short bowel syndrome is the most common form of intestinal failure and results in inability to maintain nutritional, fluid, and electrolyte status while consuming a regular diet. Nutrition interventions to treat short bowel syndrome include enteral and parenteral nutrition, intestinal rehabilitation techniques to enhance absorptive capacity of remnant bowel, and surgical reconstruction designed to provide more surface area for absorption. These therapies are interrelated services to restore n...

Fish Oil for Inflammatory Bowel Disease
Dietary supplements have increasingly gained popularity over the years not only to replete micronutrient deficiencies but for their use in treatment of disease. The popularity of dietary supplements for inflammatory bowel diseases (IBD) arises from their perceived ease of use, potential disease-modifying benefits, and perceived safety. Overall, randomized controlled trials have not consistently shown a benefit of fish oil for the maintenance of remission with Crohn ’s disease. The inconsistency of these findings highlights the need for more studies that are powered to clarify the context in which omega-3 fatty acids migh...

Nutrition in the Management of Inflammatory Bowel Diseases
This article reviews the role of nutrition in inflammatory bowel disease, including appropriate nutrition screening in this at-risk population, and associated micronutrient deficiencies. We provide recommendations on dosing supplementation. We briefly review diet as a risk factor for inflammatory bowel disease and the currently proposed published dietary intervention studies. Precision Medicine and Obesity
The highly variable response to obesity therapies justifies the search for treatment strategies that are best suited to individual patients to enhance their effectiveness and tolerability via precision medicine. Precision medicine development in recent years has been driven by the emergence of powerful methods to characterize patients ( “omic” assays). Current available information has revealed that there are numerous intermediary processes that contribute to obesity and have provided a framework for partially comprehending the mechanisms behind the heterogeneity of obesity and its clinical consequences. Some of these ...

The Life-Long Role of Nutrition on the Gut Microbiome and Gastrointestinal Disease
Bacterial colonization of the intestines occurs during the first 2  years of life. Homeostasis of the gut microbiome is established to foster normal intestinal immune development for adulthood. Derangements in this process can interfere with immune function and increase an individual’s risk for gastrointestinal disorders. We discuss the role of diet and the micr obiome on the onset of such disorders. We examine how micronutrients, prebiotics, and probiotics modulate disease pathogenesis. We discuss how diet and abnormal microbial colonization impact extraintestinal organs. Understanding the communication of nutrition an...

Nutrition Tools for the Practicing Gastroenterologist
There are several online tools, books, and applications available to enhance the application of nutrition interventions for gastroenterologists and patients with gastrointestinal (GI) disorders. Nutritional health may be compromised in GI patients because of the nature of the disease impacting use of nutritional substrates or reducing oral intake. Protein-calorie malnutrition can result from insufficient intake, malabsorption of nutrients, and increased energy expenditure, all of which can occur in certain GI conditions. Eating disorders and disordered eating, and food insecurity, also impact nutritional status. Therapeuti...

Nutritional Considerations in the Hospital Setting
Malnutrition and issues of nutrition are common in hospitalized patients. Identifying patients at nutritional risk can help to improve hospital-related outcomes. Specialized nutritional support in the form of oral nutritional supplementation, enteral nutrition, and parenteral nutrition is essential to meeting the nutritional needs of many patients. Disease-specific nutritional considerations are fundamental to the quality care of hospitalized patients. Many vitamin, macronutrient, and micronutrient deficiencies are relevant in hospital setting. Irritable Bowel Syndrome
Irritable bowel syndrome (IBS) affects 10% to 15% of the population and often is difficult to treat with available pharmacologic agents. Dietary therapies for IBS are of particular interest because up to 90% of IBS patients exclude certain foods to improve their gastrointestinal symptoms. Among the available dietary interventions for IBS, the low FODMAP diet has the greatest evidence for efficacy. Although dietary therapies rapidly are becoming first-line treatment of IBS, gastroenterologists need to be aware of the negative effects of prescribing restrictive diets and red flag symptoms of maladaptive eating patterns. (Sou...

Dietary Management of Eosinophilic Esophagitis
An alternative to pharmacologic management of eosinophilic esophagitis, elimination of food antigens for diet therapy is an effective first-line treatment strategy to induce and maintain symptomatic, histologic, and endoscopic disease remission. The 3 dietary strategies for eosinophilic esophagitis include elemental diet, empiric elimination diet, and targeted elimination diet. We review the studies supporting various diet therapy strategies, practical considerations and challenges for applying an elimination diet, and novel testing to identify triggers and optimize food reintroduction. The Inextricable Relationship Between Nutrition and Gastrointestinal Health
The primary raison d ’être of the digestive system is the processing, metabolism, and absorption of nutrients. As such, gastrointestinal health and disease strongly drive the body’s nutritional status. The presence of intestinal inflammation in inflammatory bowel disease (IBD) or celiac disease, or a deficit in int estinal surface area in short bowel syndrome, often leads to nutrient malabsorption. Pancreatitis and compromise in the synthesis or release of pancreatic enzymes cause maldigestion of macronutrients. (Source: Gastroenterology Clinics of North America)
