Interaction Between Tacrolimus and Proton Pump Inhibitor in a Kidney Transplant Recipient
Exp Clin Transplant. 2024 Jan;22(Suppl 1):354-355. doi: 10.6002/ect.MESOT2023.P89.ABSTRACTWe aimed to present a drug monitoring profile of tacrolimus and proton pump inhibitor coadministration in a 23-year-old male patient with a history of high blood pressure who underwent kidney transplant. The patient's serum trough levels of tacrolimus were in the therapeutic range until omeprazole 20 mg daily was prescribed. Tacrolimus trough serum level increased to 29.5 ng/mL under the same daily dose and to 13.9 ng/mL after tacrolimus daily dose was decreased to 6 mg/day. This increase in tacrolimus serum level was behind a renal f...

Drug-Drug Interaction between Tacrolimus and Fluconazole in a Kidney Transplant Recipient
We report a case of tacrolimus and fluconazole drug-drug interaction in a 20-year-old female kidney transplant recipient with stable kidney function. The patient's tacrolimus blood concentrations were in the therapeutic range until fluconazole was administrated for Candida albicans infection, on day 58 posttransplant. Tacrolimus blood concentration increased by 125% (18.4 ng/mL) on day 79 and by 212% (25.4 ng/mL) on day 84 posttransplant. On day 92, tacrolimus trough blood concentration returned to the therapeutic range (5.6 ng/mL), with decrease of tacrolimus daily dose by 50% (to 4 mg). After fluconazole withdrawal, the ...

A Different Perspective on the COVID-19 Pandemic: Origin of the Outbreak (Part 1)
Exp Clin Transplant. 2024 Jan;22(Suppl 2):1-14. doi: 10.6002/ect.2023.0074.ABSTRACTIn December 2019, an outbreak caused by novel coronavirus 2019 (COVID-19) started in Wuhan, China. After extensive speculation about a causative agent, a novel betacoronavirus, in the same family as SARS-CoV and MERS-CoV, was identified via next-generation sequencing from samples received from several pneumonia patients. Ribonucleic acid extracted from the patient's bronchoalveolar lavage fluid was used as a template to clone and sequence the genome of SARS-CoV-2. Shortly after, the World Health Organization announced a worldwide pandemic. H...

A Different Perspective on the COVID-19 Pandemic: Validity of COVID-19 Testing and Protective Nonpharmaceutical Interventions (Part 2)
Exp Clin Transplant. 2024 Jan;22(Suppl 2):15-32. doi: 10.6002/ect.2023.0130.ABSTRACTThe causative agent for COVID-19 was identified by next-generation sequencing from samples received from several pneumonia patients. The detection of the coronavirus was done through real-time polymerase chain reaction analyses performed from plasma sample. Ribonucleic acid extracted from a patient's bronchoalveolar lavage fluid was used as a template to clone and sequence a genome from lineage B of the betacoronavirus. A polymerase chain reaction test was considered positive using cycle threshold above 34 for detected samples. Tests with t...

A Different Perspective of COVID-19 Pandemic: Efficacy and Safety of mRNA Vaccines in Immunocompetent and Immunocompromised Individuals (Part 3)
Exp Clin Transplant. 2024 Jan;22(Suppl 2):33-62. doi: 10.6002/ect.2023.0132.ABSTRACTShortly after the declaration of the pandemic, several anti-SARS-CoV-2 vaccines entered phase 3 clinical trials. One year later, Pfizer-BioNTech and Moderna published their initial trial results, encompassing a 2-month follow-up. The studies were only designed to test whether the vaccines were effective and safe on the short-term.The vaccine's efficacy in the 2 trials was defined as relative risk reduction instead of absolute risk reduction, a more accurate determinant of vaccine effectiveness in the real world. These studies were not desig...

Evaluation of Imaging Findings in Patients with Pretransplant and Posttransplant Tertiary Hyperparathyroidism
CONCLUSIONS: Higher numbers of detectable glands and the presence of parathyroid autonomy were more common in the pretransplant group. This might be explained by parathyroid gland involution after transplant. The results may also suggest that factors other than autonomy are responsible for posttransplant tertiary hyperparathyroidism.PMID:38385382 | DOI:10.6002/ect.MESOT2023.O16 (Source: Experimental and Clinical Transplantation : official journal of the Middle East Society for Organ Transplantation)

Efficacy of mTOR Inhibitors and Intravenous Immunoglobulin for Treatment of Polyoma BK Nephropathy in Kidney Transplant Recipients: A Biopsy-Proven Study
CONCLUSIONS: In patients with polyoma BK virus nephropathy, clearance of viremia and SV40 should not be the sole outcomes to obtain. Aggressive reductions in maintenance immunosuppression and switching to double-drug therapy combined with high-dose intravenous immunoglobulin leads to high rates of graft loss/rejection and sequalae of chronic histological changes.PMID:38385385 | DOI:10.6002/ect.MESOT2023.O29 (Source: Experimental and Clinical Transplantation : official journal of the Middle East Society for Organ Transplantation)

An Emerging Issue: Carbapenem-Resistant Enterobacteriaceae in Solid-Organ Transplantation
CONCLUSIONS: Carbapenem resistance increased significantly over the years. Bacteremia 48 hours after hospitalization and hospitalization within the past 3 months were determined to be risk factors for carbapenem resistance. Carbapenem-resistant infections are still nosocomial infections. Patients should be hospitalized for as a short time as possible, and both patients and their physicians should follow infection control and prevention methods.PMID:38385389 | DOI:10.6002/ect.MESOT2023.O38 (Source: Experimental and Clinical Transplantation : official journal of the Middle East Society for Organ Transplantation)
