Neonatal Diabetes - Same disease, same gene, different outcomes.
We present 2 case reports of patients that, despite having the same presenting condition and being later found to have the same mutated gene, had significantly different outcomes. While one illustrates the importance of genetic testing on therapeutic choices, the other shows the test's limitation when it comes to predicting the patients' final outcome - showing that even the most prevalent cause of permanent NDM can cause transient forms of the condition. Thyroid dysfunction in patients with diabetic retinopathy.
Abstract Thyroid disease and diabetes mellitus (DM) are the most common endocrinopathies in clinical practice that interact with each other. On the one hand, thyroid hormones regulate carbohydrate metabolism and pancreas functions, and on the other hand DM affects the function and work of the thyroid gland. Diabetic retinopathy (DR) is a highly specific neurovascular complication of both type 1 and type 2 DM, which is a significant cause of vision loss on a global scale. In DM, the internal blood-retinal barrier is the earliest to be damaged, and changes in neuroretina result from the loss of its adaptation to met...

Peripheral levels of selected adipokines in patients with newly diagnosed multiple sclerosis.
Abstract Multiple sclerosis (MS) is an autoimmune disease of the central nervous system. The exact etiology is unknown. However, genetic and environmental factors are suggested to be involved in the pathogenesis of MS. Improper diet resulting in obesity and metabolic syndrome can contribute to this disease. Adipokines, secreted by adipose tissue, are linking metabolism and immune system. We aimed to assess plasma levels of selected adipokines in newly diagnosed, treatment-naïve individuals with multiple sclerosis. Our group comprised 58 individuals (31 MS patients and 27 controls, matched for age and BMI) wit...

Paediatric Cushing's disease - a literature review of epidemiology, pathogenesis, clinical symptoms, and diagnostics.
Abstract Cushing's disease (CD) is characterised by excess production of adrenocorticotropic hormone (ACTH) by a pituitary corticotroph adenoma, which results in hypercortisolaemia. CD is extremely rare in the paediatric population, and few paediatric endocrinology centres have experience in diagnosing and treating this disease. The clinical presentation of hypercortisolaemia is variable, so proper and rapid diagnosis of CD is often challenging. The molecular pathogenesis of CD was largely unknown until recently. The latest research has revealed somatic mutations in the USP8 gene as the most common pathogenic mole...

Pituitary tumours in MEN1 syndrome - the new insight into the diagnosis and treatment.
Abstract Pituitary tumours are a common pathology affecting 15-20% of the population. Only about 1‰ of adenomas are clinically manifested; among them, about two/thirds are hormonally active, most often secreting prolactin or growth hormone. Pituitary tumours are mainly an isolated pathology, without any genetic background. However, the latest studies pay special attention to the possibility of developing an adenoma as a result of genetic mutation. Among pituitary adenomas, the leading group of genetically determined lesions is related to a mutation in AIP or MEN1, followed by PRKAR1A, GRP101, DICER, and SDH...

Clinical implications of GWAS variants associated with differentiated thyroid cancer.
Abstract The genetic risk of differentiated thyroid cancer (DTC) probably consists of multiple low-penetrance, single-nucleotide polymorphisms (SNP). Such markers are difficult to uncover by linkage analysis but can be revealed by association studies. Genome-wide association studies (GWASs) have uncovered 31 SNPs associated with DTC. These markers carry a low to moderate risk for DTC, but their cumulative effect increases with each successive risk allele. These data support the important contribution of low penetrance variants in the pathogenesis of DTC. Contrary to somatic mutations such as BRAFV600E, germline va...

The aldosterone index could be used to diagnose the dominant gland in primary aldosteronism - a retrospective study.
Abstract Failed cannulation in the right adrenal vein, which makes the sampling results in the contralateral vein and inferior vena cava (IVC) nonsense, is the main obstacle of using adrenal vein sampling (AVS) in the lateralization diagnosis in primary aldosteronism (PA). We performed a retrospective study to evaluate the specificity and sensitivity of using Aldosterone index (AI) in PA lateralization diagnosis. We enrolled 116 patients who were diagnosed with PA and then underwent AVS in West China Hospital of Sichuan University from April 2015 to April 2017. AI, calculated by dividing the aldosterone concentrat...

Acute suppurative thyroiditis with thyroid metastasis from esophageal cancer.
CONCLUSION: This is the first case of AST complicated with thyroid metastasis from easophageal cancer and it throws new light on AST and thyroid metastasis for physicians. When patient present to hospital because of fever, neck pain and swelling,AST should be alarmed. The diagnosis of AST should be based on high clinical suspicion. In addition, thyroid cancer have to be taken into our account, especially when antibiotic therapy is ineffective.Notably, except for primary thyroid cancer, thyroid metastasis should be alarmed. Effects of levothyroxine therapy on bone mineral density and bone turnover markers in premenopausal women with thyroid cancer after thyroidectomy.
CONCLUSIONS: We found high prevalence of osteopenia among premenopausal women who received long-term LT4 therapy for DTC after thyroidectomy. Long-term TSH suppression therapy was an significant risk factor for decreased bone strength mainly by increasing bone turnover. Excess weight loss in a patient with type 2 diabetes mellitus treated with an SGLT- 2 inhibitor.
We present a 62-year-old obese male patient with a 12-year history of type 2 diabetes and poor glycemic control: HbA1c 12% (108 mmol/mol). Patient was admitted to Diabetology Department to optimize diabetes treatment. After diabetes treatment modification (among others an SGLT -2 inhibitor adding) we observed excess weight loss - 21 kg within 180 days. In our opinion, progressive weight loss can not be associated with the change in lifestyle or the increase in physical activity but with the mechanism of drug action. Does it result from the hyper-reactivity of SGLT-2 or the mechanism of drug action that has not been discove...

Opioid-like peptides and ghrelin mitigation of sleeve gastrectomy results depends on obesity level.
CONCLUSIONS: The activity of endogenous peptides in bariatric patients is connected with the obesity degree. Ghrelin level increases are negatively correlated with native Met-enkephalin changes shortly after bariatric surgery. The interplay of ghrelin and opioids might be consider as a predictor of postoperative weight loss success. PMID: 31681979 [PubMed - as supplied by publisher] (Source: Endokrynologia Polska)

Assessment of selected adipocytokines in obese women with postmenopausal osteoporosis.
CONCLUSIONS: Higher leptin concentration and a positive correlation with BMI was confirmed in obese postmenopausal women with osteoporosis. It was also demonstrated that BMD increases with growing BMI. No effect of obesity on the secretion of adiponectin or resistin in women with postmenopausal osteoporosis was found. From among the investigated adipocytokines, only leptin can be considered a bone tissue protective factor in postmenopausal women.
