Applying Geriatric Principles to Hazardous Drinking in Older Adults
Older adults continue to drink as they age. Aging changes alcohol kinetics just as with any other drug. Older adults have increased sensitivity to acute alcohol intake that accounts for the increased risk of falls, traffic accidents, and other injury. The Annual Medicare Wellness Exam is an excellent opportunity to introduce screening for unsafe drinking along with accumulated risks and deficits of aging. Older adults have responded well to brief interventions for unhealthy drinking. In the presence of alcohol use disorder or serious comorbidity including psychiatric illness, referral to specialized multidisciplinary care ...

The Science of Frailty
Frailty is an important clinical syndrome of age-related decline in physiologic reserve and increased vulnerability. In older adults, frailty leads to progressive multisystem decline and increased adverse clinical outcomes. The pathophysiology of frailty is hypothesized to be driven by dysregulation of neuroendocrine, inflammatory, and metabolic pathways. Sex-specific differences in the prevalence of frailty have been observed. Treatment interventions of geriatric care can be applied to the care of frail older women with these differences in mind. As additional evidence regarding sex-specific differences in frailty emerges...

Abuse/Misuse of Prescription Medications in Older Adults
The world population is aging due to increasing life expectancy. The rate of drug use increases, and inappropriate prescribing is frequently encountered with advancing age. In addition, misuse and abuse of prescription drugs is a serious problem in older adults. It is challenging to detect substance and drug abuse in older patients because it may have fewer consequences in social, legal, and occupational fields. However, there is not enough information about the screening, evaluation, diagnosis, and treatment of abuse. Therefore, the awareness of health care professionals and others involved in older patients ’ care shou...

Aging and Women ’s Health
Older women outnumber older men in the United States and may experience a range of physical, cognitive, social, and emotional challenges. The “Geriatric 5 Ms”—Multicomplexity, Mind, Mobility, Medications, and What Matters Most—provide a useful framework for understanding and addressing the health and wellness needs of older women in the clinic. The National Institute on Aging, a component of the US National Institutes of Health, supports a vibrant program of aging research with many immediately implementable findings and useful resources for the busy clinician. The Psychosocial Effects of Frailty on Women
Certain psychosocial elements, such as depression, anxiety, stress, lack of social support, and loneliness, should be considered as part of frailty. Women are more likely to be frail toward the end of life, because they live longer and are less likely to develop diseases with abrupt ends. Women are also more prone to develop psychosocial elements associated with frailty because of their lifetime stressors, poverty, and loneliness at the end of life. Clinicians should recognize this phenomenon and create early interventions to ensure women are able to live according to their preferences during the last part of their lives. ...
