Global Geriatric Palliative Care
Global palliative medicine is a priority for global health. The aging world population lives with multiple chronic diseases and malignancies that often lead to debility, morbidity, mortality, and decreased quality of life. In the United States, 68% of adults aged older than 65 years live with 2 or more chronic conditions. Endeavors to improve access to palliative care for seniors are ongoing within “age-friendly health systems.” This review article aims to provide an overview of the present state of global geriatric palliative care and to identify potential areas for future improvement. Advance Care Planning in the Geriatrics Clinic
Advance care planning (ACP) is a process that allows individuals to express their health-care preferences and make decisions about their future medical care. Clinicians practicing in a Geriatrics clinic or with many patients who are aged 65  years or older have a unique opportunity to discuss patients’ goals of care. ACP is particularly important for older adults, who may be facing serious health issues and/or end-of-life decisions. This review article will provide an overview of the importance of ACP in the geriatrics clinic, discu ss the barriers to implementation, and explore strategies for successful integration.. (...

Advancing the Vision of Palliative Care for the Older Adult
Aging is not a serious illness; it is a natural part of life. In this current era where ageism is openly promoted and practiced in our society, it is important to not conflate the process of natural aging with a disease. However, as treatments for serious illnesses have advanced and can provide longer lives, people, including our elders, may live with serious illnesses longer. Illness, like cancer, has become a disease of aging. It is important for primary care providers, geriatricians, and palliative care specialists to be prepared to meet the palliative care needs of these patients and families. Trauma (Excluding Falls) in the Older Adult
Trauma in the older adult will increasingly become important to emergency physicians hoping to optimize their patient care. The geriatric patient population possesses higher rates of comorbidities that increase their risk for trauma and make their care more challenging. By considering the nuances that accompany the critical stabilization and injury-specific management of geriatric trauma patients, emergency physicians can decrease the prevalence of adverse outcomes. Elder Mistreatment
Elder mistreatment is experienced by 5% to 15% of community-dwelling older adults each year. An emergency department (ED) encounter offers an important opportunity to identify elder mistreatment and initiate intervention. Strategies to improve detection of elder mistreatment include identifying high-risk patients; recognizing suggestive findings from the history, physical examination, imaging, and laboratory tests; and/or using screening tools. ED management of elder mistreatment includes addressing acute issues, maximizing the patient ’s safety, and reporting to the authorities when appropriate. (Source: Clinics in Geriatric Medicine)

Emergency Department-to-Community Transitions of Care
This article describes emergency department (ED)-to-community care transitions for older adults and associated challenges, measurement, proven efficacious and effective interventions, and policy considerations. Older adults experiencing social isolation and impairments in functional status or cognition represent unique populations that are particularly at risk during ED-to-community transitions of care and may benefit from targeted intervention implementation. Future efforts should target optimizing screening techniques to identify those at risk, developing and validating patient-centered outcome measures, and using policy...

Assessing Physiologic Reserve and Frailty in the Older Emergency Department Patient
Older patients are more vulnerable to acute illness or injury because of reduced physiologic reserve associated with aging. Therefore, their assessment in the emergency department (ED) should include not only vital signs and their baseline values but also changes that reflect physiologic reserve, such as mobility, mental status, and frailty. Combining aggregated vitals sign scores and frailty might improve risk stratification in the ED. Implementing these changes in ED assessment may require the introduction of senior-friendly processes to ensure ED treatment is appropriate to the older patients ’ immediate discomfort, p...
