Putative associations between inflammatory biomarkers, obesity, and obstructive sleep apnea
CONCLUSIONS: OSA patients exhibit increased levels of inflammatory mediators that do not appear to be associated with polysomnographic measures, but exhibit positive correlation with the degree of adiposity. (Source: Annals of Thoracic Medicine)

Drug susceptibility profiling of pulmonary Mycobacterium kansasii and its correlation with treatment outcome
CONCLUSION: There was a poor correlation between the treatment outcome and the resistance pattern of the antibiotics tested. With increasing numbers of NTM being reported, early and correct identification of NTM species is essential for the prompt initiation of appropriate treatment to achieve better outcome. Bilevel positive airway pressure ventilation for non-COPD acute hypercapnic respiratory failure patients: A systematic review and meta-analysis
Bandar M Faqihi, Samuel P Trethewey, Julien Morlet, Dhruv Parekh, Alice M TurnerAnnals of Thoracic Medicine 2021 16(4):306-322 The effectiveness of bi-level positive airway pressure (BiPAP) in patients with acute hypercapnic respiratory failure (AHRF) due to etiologies other than chronic obstructive pulmonary disease (COPD) is unclear. To systematically review the evidence regarding the effectiveness of BiPAP in non-COPD patients with AHRF. The Cochrane Library, MEDLINE, EMBASE, and CINAHL Plus were searched according to prespecified criteria (PROSPERO-CRD42018089875). Randomized controlled trials (RCTs) assessing the eff...

Pulmonary rehabilitation for COPD: A narrative review and call for further implementation in Saudi Arabia
Abdulelah M Aldhahir, Saeed M Alghamdi, Jaber S Alqahtani, Khaled A Alqahtani, Ahmed M Al Rajah, Bedor S Alkhathlan, Sally J Singh, Swapna Mandal, John R HurstAnnals of Thoracic Medicine 2021 16(4):299-305 Chronic obstructive pulmonary disease (COPD) is a common, preventable, and treatable condition, in which outcomes can be improved with careful management. Pulmonary rehabilitation (PR) comprises exercise and education, delivered by multidisciplinary teams. PR is a cost-effective management strategy in COPD patients which improves exercise performance, reduces dyspnea, reduces the risk of exacerbation, and improves healt...

Fatal rapidly progressive interstitial lung disease in a patient with amyopathic dermatomyositis
Abdullah Mobeireek, Walter Conca, Shamayel Mohammed, Fahad AlObaid, Mohammad AlHajjiAnnals of Thoracic Medicine 2021 16(3):294-298 A wide spectrum of interstitial lung disease (ID) is common and is a well-established manifestation of idiopathic inflammatory myopathies. Yet, till now, the pathogenetic mechanisms are still poorly understood, classification is evolving and prognosis is variable. A refractory and rapidly aggressive form of interstitial lung disease (RPILD) associated with dermatomyositis (DM) with minimal muscle weakness and normal creatine kinase (termed clinically amyopathic DM) is increasingly being recogn...

Molecular detection of plasmid-derived AmpC & #946;-lactamase among clinical strains of Enterobacteriaceae in Bahrain
CONCLUSIONS: Plasmid-mediated spread of AmpC is present in clinically relevant Enterobacteriaceae species in Bahrain. Rational antimicrobial therapy against these multidrug-resistant organisms and continued surveillance of antimicrobial resistance mechanisms among the clinical isolates are recommended for optimal patient care. Clinical profiles associated with SARS-CoV-2 infection and complications from coronavirus disease-2019 in children from a national registry in Saudi Arabia
CONCLUSIONS: COVID-19 is usually a milder disease in children. Although having preexisting medical conditions was linked to a longer hospitalization, it was not associated with worse outcome. Continuous surveillance will allow additional characterization of the burden and outcomes of pediatric COVID-19-associated hospitalizations. (Source: Annals of Thoracic Medicine)

Pulmonary neuroendocrine carcinoid tumors: Is there a predictive role to the Ki 67 index ?
CONCLUSIONS: Histological type is the best predictor of prognosis in the carcinoid tumor group. In the AC subgroup, the marginal association between mortality, frequency of relapses and Ki values 67 ≥ 1.18 has clinical relevance future analyses are required to determine the real predictive value of this variable. Point-of-care diagnostic lung ultrasound is highly applicable to the practice of medicine in Saudi Arabia but the current skills gap limits its use
CONCLUSIONS: Whilst POCDLUS is applicable to IM practice in Saudi Arabia, the significant skills gap preclude the provision of a POCDLUS service. As COVID-19 can cause an interstitial syndrome, our pandemic preparation response should include POCDLUS training. The current study is supported by a similar Canadian study and the international standardisation of POCDLUS training may be feasible. The findings of the current study may facilitate the development of POCDLUS training programs for internists throughout Saudi Arabia. (Source: Annals of Thoracic Medicine)

Post-COVID-19 functional status: Relation to age, smoking, hospitalization, and previous comorbidities
CONCLUSIONS: Most of the COVID-19 recovered cases have diverse degrees of functional restrictions ranging from negligible to severe based on PCFS. These restrictions were affected by age, gender, periodic influenza vaccination, smoking, duration since symptoms onset, need for oxygen or ICU admittance, and finally the presence of coexisting comorbidity. Small-airway disease and its reversibility in human immunodeficiency virus-infected children on highly active antiretroviral therapy: A cross-sectional study in an African setting

The efficacy of airway pressure release ventilation in acute respiratory distress syndrome adult patients: A meta-analysis of clinical trials
CONCLUSION: In this study, using the APRV mode may improve oxygenation on day 3 and contribute to reducing the LOS in ICU. However, it is difficult to draw a clinical message about APRV, and well-designed clinical trials are required to investigate this issue. (Source: Annals of Thoracic Medicine)
