Antipsychotics & hypothermia
What are your experiences with patients having HYPOthermia (not hyperthermia/NMS) as a side effect of antipsychotic therapy? I encountered my first case where this seems to be happening and am not sure what to make of it. From what little literature we could find it seems very rare and to be most common in the time periods after initiation or dose change, neither of which apply to my patient. I am a reapplicant from last year. I was able to get some good app growth in the meantime and am reapplying again this year. However, right now I have zero IIs. By this time last year I had 2 of my 3 invites and I had a worse application and a worse school list. I’m really worried and also confused as to why. Cardiac arrest temperature management
After the article in NEJM from June 17, "Hypothermia vs Normothermia..." is there anything left for holding a patient's post-arrest temperature to 32-34 or 36 post arrest? So much has evolved since the initial 32-34 study, including the 36 study, some data showing no benefit to cooling in-hospital arrest, then Hyperion. Now we have this! What should practice be now? intrathecal morphine induced hypothermia
Y'all ever seen this? Interesting case this last month. Healthy g1p0 undergoing elective cesarean. Standard spinal dose of 12mg but, 200mcg duramorph, 10mcg feet, and 100mcg epi. Not acute issues during the procedure. In the pacu, pt's temperature drops to 94-95 degrees F and remains there despite forced air warming gown/blanket thingy, warmed fluids etc. Pt's temp eventually returned to normal after several hours. The end of therapeutic hypothermia post cardiac arrest?
There is an editorial in the BMJ this week covering the Targeted Temperature management trial (TTM) trial of cooling to 33 degrees versus 36 degrees in cardiac arrest survivors who remain comatose post resuscitation.
