From FM to IM to speciality
I'm in my third year of a FM program, 2 months from graduation. I find that primary care is not my cup of tea, I had to take this residency because it was a back up. Before this I did one year as a transitional year. What, if at all possible, steps I should take to apply [and get into] an IM residency? I want to to do specifically do IM and not just practice as a Hospitalist in a FM role because I want to further specialize in IM sub-specialties. Thank you (Source: Student Doctor Network)

Hospitalist job offer for a new grad. What are your thoughts?
I am graduating an FM residency program with a strong inpatient training component. The offer: Hospitalist at a small community hospital (100 beds) in a small town (30,000) 45 minutes outside of a small city (150,000) 7 on 7 off 7am to 7pm (days only, no nights, no call). Two weeks vacation. Census is apparently between 5 and 20 but I am told the average is around 12 each. There are only two hospitalists working at a time during the day. We cover codes. Admissions are rotated between the... Hospitalist job offer for a new grad. What are your thoughts? (Source: Student Doctor Network)

How to keep options open in residency with minimum effort
To make it short, I'll be starting intern year soon at a mid tier university program. My plan right now is to forego fellowship and go straight into hospitalist medicine. The only way I see that plan changing is if hospitalist job market crashes while the lucrative sub specialties remain lucrative. In that case, I may have no choice but to stick around for 3 more years and do either cards or GI. So the thing I am trying to figure out right now is how hard would I have to hustle to have a... help me decide: PCP vs heme/onc
Hi everybody, I am a current PGY2 and am struggling between solid oncology vs primary care. Inpatient medicine burns me out due to heavy page burden and having to direct my attention to many different things at once. I feel more relaxed when I can devote myself to one thing at a time. Thus, hospitalist work is out of the question for me. I need help figuring out what is the best fit for me. I want a job where I can feel fulfilled and not burnt out. I don't care about salary/ prestige. I... help me decide: PCP vs heme/onc (Source: Student Doctor Network)

Graduating IM Resident, thinking about Anesthesia
Hello, everyone! Currently, I'm PGY-3 IM Resident hoping to graduate this summer. Throughout my training, I have had an interest in Critical Care and enjoyed Managing airways during my Anesthesia rotation at the end of my PGY-2 year. I really enjoyed Anesthesia. Now I am having trouble deciding to what route should I go for. I can either apply for Pulmonary/CC or just CC fellowship this year and work as a Hospitalist for one year. Or, I can just apply for Anesthesia and do one year of... Graduating IM Resident, thinking about Anesthesia (Source: Student Doctor Network)

Should We Code COVID19 Patients?
GeneralVeers said: They wanted me to intubate a 96-year old COVID positive patient last night. 96!!! He was DNR/DNI but the spineless hospitalist made him "full code" because in his anxiety and hypoxia the patient apparently requested he be intubated. Fortunately his ABG wasn't too bad, and I was able to punt this one so I didn't have to do something I considered unethical. Click to expand... ShockIndex said: The fact that... Click to expand... Anyone do FM then IM residency?
Or really, any other residency to IM? Then possibly fellowship? I'm a PGY1 at an FM program with strong inpatient medicine in a major city. Turns out I love medicine wards and critical care way more as a resident than as a med student. I know I can do hospitalist work as FM but after speaking with my mentors, it sounds like the pickings are getting slimmer in my area as hospitals/groups prefer IM trained docs. Is there anyone here who's completed FM training then did a second residency in... Anyone do FM then IM residency? (Source: Student Doctor Network)

Do you eat in the Doctor's Lounge?
Do you guys eat in the doctor's lounge during this Covid-19 pandemic? I kind of want to stay away from the pulmonologists and hospitalists so I've stopped eating there. Any of you taking the same precautions? (Source: Student Doctor Network)

M-F Hospitalist
I just wanted some advice. I have a unique opportunity to take a job in a nearby southern state, working Monday-Friday as full time hospitalist (Average 10pts/day). I will also be covering the medical aspect of a Geri psych unit that averages about 5pts. They only have to be seen twice per week and prn. Only downside is I am the sole hospitalist 24hrs a day Monday-Friday so I take all night call, but only have to attend emergencies at night, no admissions. ED docs cover hospital on weekends... Second residency in Emergency Medicine
Hi everyone, I'm interested in pursuing a second residency in Emergency Medicine. I completed an Internal Medicine residency in 2017 and I have been working as a hospitalist since then. I'm ABIM board certified. I would like to be a more versatile doc - peds, OB, performing lifesaving procedures, etc. I'm planning to apply this coming September but, unfortunately, I don't have any letters of recommendation from Emergency Medicine physicians. Would my application be considered if I don't... Second residency in Emergency Medicine (Source: Student Doctor Network)

Rural - Specialist F/u vs PCP F/u
1 - Hey y'all, if you have an osteo or endocarditis patient, and you're a hospitalist in a rural area without ID around for >1 hour, do you end up sending them to follow up with a local PCP to monitor interval CRP/ESR (osteo)? Or do you make sure they see an ID doc even if the distance is significant? 2 - If you treat these patients without an ID consult (lack of ID at your hospital, etc), does this make a significant difference in your choice? Why does everyone lose their minds over FM having a wide scope of practice, but lets midlevels do literally anything?
Not intended to be a midlevel thread. I'm genuinely curious why people in medicine, especially in FM, think that we should have some sort of narrow scope of practice. Anything with high acuity? Nope. Managing less common diagnoses? Nope. (have to refer it out, so they're see by the endocrinology PA!) Ob? Nope, we aren't qualified. Scopes? Nah... but totally okay if we let "GI NPs" do it. C-sections? No. But midlevels in UK can train to do "basic surgeries." Hospitalist? No, we aren't... hi all - was very competitive residency applicant. switching into IM. staying at intern year program, which is a community program that does not have any in-house fellowship positions. Ghosted After Interview
I am a hospitalist and I have been shopping around for a new job to relocate to a bigger city with my family. My first job out of residency, I received the offer the very next day after my site visit interview. So far, I have been on three site visit interviews since started looking for a new job. I received offers from two of the onsite interviews, but I declined due to the hospitals not being able to meet my expectation on the contract negotiation. The third site visit did not give me an...
