Ascension outsources Illinois hospitalist job
Ascension will outsource Illinois hospitalist functions to PE-backed staffing firm Beginning June 1, the Catholic health system will cede responsibility of hospitalist functions to staffing firm SCP Health. www.healthcaredive.com This does not feel good about hospitalist job market and career outlook. Possible to match pulm crit?
US MD from university residency program, with average board scores and low research (4-5 case report presentations at national conferences, published in conference proceedings). I decided on PCCM very late, and therefore will be doing a point of care ultrasound fellowship next year. I plan to get involved in POCUS research and apply immediately after completion of ultrasound fellowship while working as a hospitalist for that year. I am open to direct critical care fellowship as well and... Read more (Source: Student Doctor Network)

Are diabetics immunocompromised in your practice?
Had some pushback last night from a hospitalist. Been out 4 years and never heard this argument before. 50's male with HTN, IDDM. Came in febrile to 102, obviously tacky. WBC 16k, lactic 2.5. Viral panel neg. Pneumonia on CT. Did all the sepsis stuff and covered with abx. The hospitalist argues with me and says they are stable and can be sent home and when I said they are immunocompromised with DM he said "almost everyone is diabetic so what" Threw me for a loop. I would never consider... Read more (Source: Student Doctor Network)

Past PIP and effect on licensing and credentialing
A few months into my first hospitalist job post-residency, an adverse outcome patient case was reviewed. While the cause of the adverse outcome was multifactorial and it is possible that extra imaging/quicker specialist consultation may not have made a difference, I was queried. Admittedly, there were points I could have improved on, like better documentation to reflect that I was aware of everything going on. Based on this, the hospital ended up Proposing a voluntary PIP. I honestly was so... Read more (Source: Student Doctor Network)

Workaholicism...
Does anyone have an workaholic gene that's perpetually activated? early in my hospitalist career I was pretty hard core about working. I wanted the money and I enjoyed the work. Fast forward now many years later.. financially I'm in a good place now, and a few years ago I realized I didn't have to be doing extra shifts and in fact mathematically it wouldn't move the needle. But I still feel sometimes the urge to work extra shifts and be productive, I still enjoy the work. Can anybody relate... Read more (Source: Student Doctor Network)

Working nights as a hospitalist
Is it possible to negotiate a contract as a hospitalist where you don't have to work any night shifts? I'm willing to work evenings and weekends. I have a health issue where I have trouble inverting my sleep cycle. And how does this differ between academics and private practice? (Source: Student Doctor Network)

Baller Hospitalists 2.0
The last thread died in 2020, and there have been 1 or 2 changes since then. So let's get a new thread going. How I envision this - if you're HM post your gig in the format below (modify it however you want). We can talk about offers and jobs and help answer questions that students, residents or hospitalists may have. I expect this thread to highlight the great deal of pay disparity in HM and let people see what makes jobs desirable versus not. What I don't want - don't turn this into a... Am I a nobody?
I recently went to a 20 yr medical school reunion. Some classmates are assitant deans and department heads, high powered specialized specialists, and big wigs in their fields, and then there's me (lowly hospitalist- glorified resident - worker bee). I've never been the envious or jealous type, but it is interesting to see. Anyone else can relate and does it bother you? Don't get me wrong, I'm happy, I've had a good career so far and I've done well due to good financial habits etc. (Source: Student Doctor Network)

HCA hospitalist
I've heard the horror stories, but what's the truth about working for an HCA hospital as a hospitalist these days? What if it's through a staffing agency rather than HCA itself? The contract looks good and the patient load is OK from what I have been told. Any help would be appreciated. (Source: Student Doctor Network)

About to sign my first attending contract; what are things I need to absolutely be sure are included in it?
About to sign my first contract to be a hospitalist. It’s with a large well known medical group. There are things that I know must be included in the contract, or that I need to be aware of. For instance, I know that tail coverage needs to be included in the malpractice insurance that is included in the contract. What other things are important to know in your contract and/or are things that I must have in there? (Source: Student Doctor Network)

Nephrology fellowship after 17 years of hospitalist work ?
I have been a hospitalist ( nocturnist) for 17 years ( nocturnist for last 9) My pay hasn’t increased in about 7 years. I work 10 nights in a month. 14 hour night shift each, so essentially one night is equal to one and half days ! I do get to spend a lot of time at home with my kids, something I greatly cherish . Now I have been thinking of applying . There are some positions unfilled in the area. First I am not sure if they will take me being so old. Second, not sure if it’s financially... Performance improvement plan, may get fired? Advice.
Am IM hospitalist rural midwest. Have not been at my job very long but have been having constant issues with nurses, APPs, to the point that two nurses were recently fired and I was placed on a performance improvement plan (PIP). The goals seem reasonable, but doing some research it seems as though a PIP is the first step toward being fired, so I want to know if anyone has experience in having a PIP, if this means I am for certain going to get fired or if I can come back from this, and... Adrenal incidentaloma - how much do y'all work it up in the hospital?
Question for hospitalists out there. CT imaging so often shows an adrenal incidentaloma. I see various practices regarding what is done, and I'm wondering what y'all's practices are. 1) Rule out hormonal activity in the hospital (aldosterone renin ratio, dexamethasone overnight suppression test, or assess metanephrines) before discharge. 2) Order repeat imaging but defer hormonal evaluation to outpatient setting 3) Defer hormonal evaluation and repeat imaging to the outpatient follow up...
