Quick prone MAC colorectal cases: what do you use?
Recently out from residency into private practice. We didn’t do these prone cases (fistula, hemorrhoids, etc) much in residency. I was just curious what you all do for them. Seems like the only painful part is the local injection. So I was thinking maybe versed and propofol would be best, but was also thinking of using remifentanil? Seems like it would wear off faster that propofol if patient went apneic. The operating time for these cases is usually less than 5 minutes. (Source: Student Doctor Network)

Anorectal varices and hemorrhoids difference
Hey, In portal hypertension, blood is shunted from the superior hemorrhoidal vein to the middle and inferior rectal veins. However, Pathoma claims that the portosystemic shunts include hemorrhoids. Is this correct? I mean, definition of hemorrhoids is dilated veins either above or below the dentate line. So it seems that anorectal varices just fall into this broad general category that is hemorrhoids. Can I reason this way or is this wrong? (Source: Student Doctor Network)

Hospital job - NOT what I was expecting
Hi guys, I need to get something off my chest. So I made the switch from retail to hospital a little over a year ago, after 8 brutal years in retail. I was hoping I would relearn all the material I forgot after school and actually apply it in practice. Frankly, I just feel like a glorified retail Rph. Yes I do some "clinical monitoring" but that literally takes 15 minutes. Mostly I am tubing up meds, checking cart fills, entering piles of phone orders, and doing mundane stuff I can...
