MKSAP: 52-year-old man with fatigue and fever
Test your medicine knowledge with the MKSAP challenge, in partnership with the American College of Physicians. A 52-year-old man is admitted to the hospital with fatigue and fever of 3 days’ duration. He is a health care worker and has a bicuspid aortic valve. He takes no medications. Blood cultures are obtained at the time of admission, and he is started on empiric vancomycin for possible endocarditis. On hospital day 2, his initial blood cultures become positive for gram-positive cocci in clusters, and on hospital day 3, his blood cultures grow methicillin-resistant Staphylococcus aureus. Susceptibility to vancomy...

FungiDB 2.3 released with 52 genomes
The latest release of FungiDB (2.3) is now live and includes 52 genomes, 11 of which are new for this release. This was a longer than expected release cycle due to reintegration with the EuPathDB software team. Programmers Raghu Ramamurthy and Edward Liaw at UC Riverside did nearly all the Fungal specific work, collaborating closely with the EuPathDB team who provided many site-specific corrections and assistance in running the workflow. This is a joint collaborative project between the UCR,Oregon State (FungiDB) and U Penn, Univ of Georgia (EuPathDB) and the work in this release was funded through grants from th...

Healthcare Update — 04-01-2013
This case report is entirely bizarre. Patient gets awarded more than $800,000 after visit to doctor resulted in incorrect diagnosis of cancer when the patient really had pneumonia and caused patient to have amputation of her foot. A trial was held on the case four years ago with a verdict in favor of the doctor, but the judge declared a mistrial because Washington State jurors were referring to the plaintiff’s Japanese attorney as “Mr. Miyagi” and were making other racist comments against him. Then, the article notes that the doctor had been disciplined by state regulators for making “erroneous diagnoses” and for...

Gram Positive Cockeye vs Cocci Explained.
Here's a letter I wrote to the National Academy of Nursing Vocabulary last week regarding my concerns about an epidemic of gram positive cockeye sweeping our hospitals.  You think the flu was bad this year?  You think C diff is bad.  You have no idea how aggressive the cockeye has been. Dear Academy, I have been a practicing hospitalist now for ten years.  I want to commend your organization's mission to improve nursing vocabulary.  Helping nurses achieve proficiency in medical terminology is important for doctor-nurse and nurse-nurse communication of critical patient information.  Quite obvi...
