Jaundice and urine color
In hemolytic anemia urine urobilinogen is increased which would mean that the amount coverted to urobilin would increase as well and urine color would at least change in intensity, so how it would be normal as if nothing changed knowing that urobilin is the pigment that gives the urine it's color? PaO2 in severe anemia
So Kaplan physio explains that both polycythemia and anemia do not affect PaO2 b/c they only affect the oxygen transported by hemoglobin not the oxygen dissolved in plasma. However, in case of severe anemia, theoretically wouldn't you observe slight increased PaO2? Since the oxygen molecules will be forced to be dissolved in plasma? Considering normal PaO2 is around 95, it may go up a bit like 97-98 mmHg right? (not exceeding 100 mmHg since it will still need to be equilibrated with the... Hey sdn, I learn a ton just by reading these forums so I just wanted to start a thread where we each list a hypersensitivity (please reference) and we'll learn as we go. I'll start Chronic gastritis/pernicious anemia - type 4 HS (pathoma pg 104, sec V.C.2.iii.) P.S. please try to list a HS that is unexpected. How can you send someone who probably was coming of anemic symptoms and bloody or tarry stools home with unstable vitals and don't even both to do a basic CBC? This is the basic mistakes that I all the time coming from these NP's. The vast majority of the time, we physicians catch their mistakes but don't let the patients, the hospital, the state know what could have happened if we didn't. How in the everloving f*ck are you supposed to memorize everything for micro/anti-microbes?
For example, in the adverse effects listed for sulfonamides, I have (from class lecture): "kernicterus, nephrotox-crystalluria, HS (cross-rxn drugs w/ S), phototox, rash, angiodema, stevens-johnson, marrow supp (anemia, granulopenia, thrombopenia), asplastic anemia (rare), hemolytic anemia (G6PDH def), anti vit K" How are you supposed to approach memorizing such a dry list of facts? Are you supposed to know every single detail on the list, and just brute force go over it repeatedly/ANKI a... Lead poisoning and FEP levels
Lead inhibits 2 enzymes in heme synthessis ;ALAD and ferrochelatase . Ferrochelatase is the last enzyme so it makes sense that protoporphyrrins can accumulate but what about ALAD ? If there is no ALAD( which is the second enzyme upstream )how can the porphyrrin ring even form ? Summary of question : 1) what is the FEP level in lead poisoning ? Also some extensions to the above question 2) what is the FEP level in sideroblastic anemia ? 3) what are the lab values for both these anemias ?...
