PCP changing medication dosage
Patient was referred by PCP and was seen once. Patient was on Xanax and changed to klonopin for panic attacks with an agreement to stop cannabis, advised to obtain complete sobriety from cannabis otherwise no benzo will be prescribed while monitoring drug screen. Patient was seen by PCP who increased dosage of benzo and provided one refill on top of it while knowing my plan on initial consult and patient admitting to continuous use of cannabis as before. I have strictly warned patient on... Read more (Source: Student Doctor Network)

Self-reflection after Quitting Medical School in 2015.
I'm 31 years old. My Xanax addiction started innocently when I was probably about 22, in 2012 -- in the prime of my life, at the time. I found myself struggling with a bit of insomnia as I was in the process of applying to medical school and taking Adderall in the name of vocational success. That's how Xanax entered my life: upon an initial and cursory complaint of difficulty sleeping. Had I known this garbage would leave me grasping at the strings of my very existence, I'd have scoffed at... Self-reflection after Quitting Medical School in 2015. (Source: Student Doctor Network)

Are benzodiazepines + marijuana ever a reasonable option for control of anxiety and depression?
I'm a pain doctor, not a psychiatrist. Have a young male patient with profound anxiety. Seeing a psychiatrist and psychiatrist is prescribing xanax 1mg TID PRN + "medical" marijuana + atomoxetine 60mg Q24hr Is this considered an appropriate medication combination? Not my specialty and want some opinions. (Source: Student Doctor Network)

A celebrity overdose death that didn ’t get much attention
I came across this today by chance but Paton Oswalt’s wife died of an accidental overdose a few years back. She was 46 and the coroners report said she died of a combination of “Adderall, Xanax, and fentanyl.” This begs two questions, why was she on this ridiculous combination and who is the moron who was prescribing this? https://www.google.com/amp/ew.com/news/2017/02/03/patton-oswalt-michelle-mcnamara-cause-of-death/amp/ (Source: Student Doctor Network)

New Forum Style Sucks
Seriously, the new forum style sucks arse... And Rick Astley? Really, the forum needs Rick Astley? The infamous Microsoft "assistant" from the 1980's? Ugh... I need a nitro and Xanax. EDIT: Yep, definitely 4-1 joke. The "Rick Astley" references above were actually references to "Rick Astley" (C l i p p y - in case SDN auto changes it in quotes too). The infamous Microsoft assistant seems to be automatically changed to Rick Astley. "Never gonna give you up..." Funny, funny. SDN has been Rick... Pharmacist in need of advice
A patient comes in with prescriptions for hydromorphone, alprazolam, carisoprodol (I also saw gabapentin on her profile) all from the same prescriber from a reputable pain management clinic. I told her I would not fill her scripts until I talked to her doctor/physician's assistant, which isn't possible since I'm staffed overnight. I normally refuse all holy trinity scripts, but this is the first time I've see that combo prescribed from that clinic (we fill a ton of their other scripts and... Tapering off Benzo's?
Do you guys have any general strategies you use? I'm getting patients who have been on Xanax their whole lives and I have made it clear to them that I will no longer be continuing the medication as they are also receiving Opioids. A few of the patients have already complained and left the clinic as their PCP's would be more than willing to refill their medications. However, I have been trying to dose them on an equivalent version of Klonopin and taper 25-50% every 2 weeks. Am I being too... Tapering off Benzo's? (Source: Student Doctor Network)

What would you do?
If you are a floater and get a script for norco, xanax, and soma, you see it’s been filled before. Would you fill it? You don’t want to lose a customer but you also don’t want to hurt the patient or get in trouble (Source: Student Doctor Network)

Personal Statement and Generalized Anxiety Disorder
Hello, I'm in the middle of writing my personal statement for next cycle. I was wondering if I should mention my 11 year struggle with Generalized Anxiety Disorder. I think it's an integral part of my journey and one that shows just how far I've come as a non trad. It does tie into the "why medicine?" question as the physician who diagnosed me is also the same physician I shadowed. I don't take medication for it. I have a prescription for Xanax for my occasional panic attacks but I keep my... Personal Statement and Generalized Anxiety Disorder (Source: Student Doctor Network)

tons of people on Adderall...
So an NP recently quit the private practice I'm at because she was looking for a job with benefits which is fine. Then....I inherited a number of her patients. Almost a good half of them on are Adderall IR as first line treatment. Nearly another half are on some sort of benzo with Xanax being relatively common. The ADHD diagnosis was thrown around pretty loosely, even applied to the guy that smokes pot daily and is getting early oxycodone refills. Interestingly when I called him and told him... tons of people on Adderall... (Source: Student Doctor Network)

Is this an example of bipolar?
A friend of mine just had his world flipped on him. His girlfriend of 4 years, with whom he has 2 children with, just left him for another man. Now, this is normal. Happens all the time. The part that struck me as odd was her back story and how she is acting now. She's been a recovery addict/addicted to Xanax for almost 8 years off and on. Now during their relationship he has told me several things that I always thought was addict behavior, such as lying, stealing, etc. Although, a couple... Is this an example of bipolar? Use of multiple benzos with stimulants
Hi all, I'm wondering if you could offer any insight about a patient who came into my pharmacy today with scripts for Ritalin 30 mg qid xanax 0.5 bid and klonopin 2mg bid. I really don't pretend to be a psych expert so i called the office and got a response of "just fill it". I didn't end up filling it but I guess my questions are where do you guys typically draw the line on titrating stimulant doses, what's the rationale behind prescribing stimulants with benzodiazepines, and what's the... The worst polypharmacy
I may have a winner for worst polypharm ever. Young (early 20's) individual comes in after being hospitalized several times out of state after being hospitalized multiple times for anxiety and "ADD." He brought in his discharge paperwork that had his d/c meds, which included, Get this -- 2 Atypical AP's (I bet you can guess what one of them is) Strattera Adderall An SNRI Xanax Restoril Valium, and Tramadol "for pain" -- without any actual physical health conditions.
