Exam Room 2: "I'll swallow this whole bottle of Aleve unless you RX."
38 yo male with cervicalgia and headaches status post ACDF C4-C5; C5-C6 cervical fusion X 2. PCP stopped opioid RX'ing "due to changes in the law." Wife reports that patient is burning through 1 bottle of Aleve every 3 days (8-12 tablets per day). Botox done elsewhere not effective. Occ NB's done elsewhere not effective. Ditto for dryineedling, acupuncture, Reiki, TPI's. Previously tolerant to oxy 60 mg QD and Soma TID. What's your next move? (Source: Student Doctor Network)

Abandoned Practices for Chronic Pain???
A lawyer asked me a couple thought-provoking questions: Lawyer: "Doctor, is there a professional consensus on the use of opioids for chronic pain?" Me: "No." Lawyer: "Doctor, has the field of pain management abandoned using opioids for treating chronic pain?" Me: "No." So, it got me thinking. What have we abandoned? How will we ever know that there is a consensus about the appropriate use of a modality based upon evidence? Everything (chiro, acupuncture, opioids, PT, psych, etc)... Khan Academy Operant Conditioning Questions
Hello all, I've never had problems with operant conditioning; however, I've missed 2 questions from Khan now pertaining to operant conditioning. One has to do with a patient finding relief from pain due to acupuncture and deciding to continue acupuncture as a result. I thought this was an example of positive punishment because acupuncture is being added in order to decrease pain; however they say negative reinforcement because pain is being removed and she decides to keep going to...
