In Reply In response to our recent review of HFrEF, Dr Dowling and colleagues correctly highlight that mechanical circulatory support devices are anticipated to play an expanding role in the management of patients with advanced heart failure, particularly as technological advancements result in development of devices with greater efficacy and more favorable safety profiles. With improvements in device technology, early identification and referral of appropriate patients to a heart failure specialist will become even more important, so that patients do not present late with multiorgan failure, rendering them noncandidates for advanced therapies.
