Repair of DeBakey Type I Acute Aortic Dissection

Acute DeBakey type I aortic dissection remains one of the most lethal cardiovascular emergencies. Surgical mortality after acute aortic dissection repair are 17-18%.1,2 The extent of repair, limited repair (i.e. hemiarch repair) vs. extended repair (i.e. total arch repair), for type I aortic dissection has been controversial. The hypothetical benefit of extended repair over limited repair with hemiarch replacement is that patients will have less distal reintervention and subsequent improved survival.
