971 every trick in the book: advanced management of refractory bleeding in a patient with roux-en-y gastric bypass

A 76-year old male with a history of a Roux-en-y gastric bypass initially presented to our ED with melenic stools and hypotension. The patient underwent an upper endoscopy without a source of bleeding identified. Over the next several months the patient had multiple admissions for gastrointestinal bleeding without a source identified. The patient came back to our hospital with abdominal pain and was found to have a perforated duodenal ulcer which was successfully managed surgically. In the following months the patient again came back to our ED with bleeding.
